10 חודשים לאחר ניתוח אולטראסוני בגישה פתוחה משמרת (Preservation Rhinoplasty).
ניתן לראות כי לפני הניתוח נצפו מספר בעיות באף: במבט קדמי – סטייה לשמאל, טיפ רחב ובולבוסי, נחיריים בולטות ואזור הקולומלה בצניחה. בפרופיל – אף נשרי ובולט קדימה, עם צניחת טיפ אך בזווית סלידה תקינה. בניתוח טופלו כל הנקודות הללו, והתוצאה לפניכם: אף קטן ועדין יותר, טיפ מעודן וגשר אף עם גלישה נשית עדינה. האף החדש יצר הרמוניה נעימה בפנים והעניק למטופלת מראה טבעי ומחמיא.

10 months after an ultrasonic open preservation rhinoplasty. Before surgery, several nasal issues were identified: on frontal view – deviation to the left, a wide and bulbous tip, prominent nostrils, and a retracted, ptotic columella. On profile – a droopy, projecting nose with tip ptosis, yet an adequate nasolabial angle. All of these issues were addressed during surgery, and the result is presented here: a smaller and more refined nose, a delicate tip, and a nasal dorsum with a soft, feminine contour. The new nasal shape created significant facial harmony and provided the patient with a natural and flattering appearance.

ניתוח אף ראשון שבוצע בעזרת שתל סחוס מצלע ושתל פאציה טמפורלית. במקרה מיוחד זה נעשה שימוש בשתלים כבר בניתוח ראשוני, בשל מורכבות המקרה.
לפני הניתוח ניתן היה לראות עיוות משמעותי בציר האף, עם סטייה בולטת של גשר האף, עצמות אף רחבות מאוד והבלטה גרמית בעיקר בצד ימין.
ניכרה אסימטרייה משמעותית בין שני צידי האף, והטיפ היה משוך לצד ימין.
מבחינה רפואית, אובחנה מחיצה מעוותת ובעלת עקמומיות ניכרת.
לאור המורכבות הוחלט לבצע את הניתוח בעזרת שתל סחוס מצלע ושתל פאציה טמפורלית, ולגשת אליו כאל ניתוח שחזורי לכל דבר ועניין.
התוצאה לפניכם – עשרה חודשים לאחר הניתוח, עם אף מאוזן, טבעי והרמוני המשתלב היטב עם תווי פניו הגבריים של המטופל.

Primary rhinoplasty performed using a rib cartilage graft and temporal fascia graft. In this particular case, grafts were required even in a primary procedure due to the complexity of the deformity.
Preoperatively, the nose demonstrated a marked deviation with significant distortion of the nasal bridge, wide nasal bones, and a pronounced bony convexity, particularly on the right side.
There was a clear asymmetry between both sides of the nose, and the nasal tip was displaced toward the right.
Medically, a severely deviated and distorted septum was diagnosed.
Given the complexity, the surgery was approached as a reconstructive case, utilizing a rib cartilage graft and temporal fascia graft for structural support and contour refinement.
The result shown here – ten months postoperatively, presenting a balanced, natural, and masculine nose that harmonizes well with the patient’s facial features.

חמישה חודשים לאחר ניתוח אף אולטרסוני בגישה פתוחה.
כפי שניתן לראות, השינוי במראה האף השפיע לא רק על הפרופורציות והיופי של הפנים – אלא גם על תחושת הביטחון והנוכחות האישית. אצל מטופלים רבים, השינוי מוביל לרצון להדגיש את המראה החדש באמצעים נוספים כמו צבע שיער, עדשות או ריסים – כחלק מתהליך כולל של התחדשות עצמית.

במבט קדמי - קיצרנו את האף, הרמנו את הטיפ הנפול ויצרנו עידון של הטיפ. בשלב זה של ההחלמה יש עדיין נפיחות ובצקת וצפוי עוד שיפור בהמשך. מבחינת גשר האף, יישרנו אותו ויצרנו קווים דורזליים נאים ומוגדרים יותר.
בפרופיל - ביצענו שינוי באזור הרדיקס – יצרנו מעבר מוגדר יותר בין המצח לאף והענקנו לאף גלישה עדינה במקום הנשריות שהייתה. קיצרנו את האף והרמנו את הטיפ.

התוצאה – אף קצר יותר, עדין והרמוני – המשלים את תווי הפנים באופן טבעי ומדגיש את הייחוד האישי של המטופלת.

Five months after an open ultrasonic rhinoplasty.
As can be seen, the transformation in the nasal structure affected not only the facial proportions and aesthetics – but also the patient’s sense of confidence and presence. For many patients, this kind of change often inspires a broader personal renewal, including subtle aesthetic adjustments such as hair color, eyelashes, or eye lenses.

From the frontal view, the nose was shortened, the bulbous tip was refined and elevated, creating a more delicate nasal definition. At this stage of recovery, residual swelling and edema are still present, and further improvement is expected over time.
Regarding the nasal dorsum, we straightened the bridge and created smooth, well-defined dorsal lines.
In the profile view, modification of the radix area established a clearer transition between the forehead and nose, giving the nasal contour a soft, natural slope instead of the previous convexity. The nose was shortened, and the tip refined and elevated.

The result – a shorter, refined, and harmonious nose – complements the patient’s facial features naturally and highlights her individual character.

אשה בת 65 שהגיעה למרפאה לייעוץ במטרה להשיג מראה צעיר ורענן יותר, תוך שמירה על מראה טבעי ולא עשוי. בשיחה הומלץ על ניתוח משולב של מתיחת פנים וניתוח לתיקון האף, על מנת ליצור מראה הרמוני ואסתטי. חשוב לזכור כי תהליכי ההזדקנות משפיעים גם על האף: קצה האף נוטה לצנוח, האף עלול להתרחב, ומראה נשרי קיים הופך לבולט יותר, מה שתורם למראה מבוגר.

A 65-year-old woman presented to the clinic for a consultation with the goal of achieving a younger and fresher appearance, while maintaining a natural and unoperated look.
During the discussion, a combined procedure of Facelift and Rhinoplasty was recommended to create a harmonious and aesthetic result. It is important to remember that the aging process also affects the nose: the nasal tip tends to droop (nasal tip ptosis), the nose may widen, and a pre-existing aquiline (hooked) appearance becomes even more pronounced, contributing to an aged facial look.
As evident in the before-and-after images, the combined surgery successfully achieved a harmonious, refined, and youthful appearance, which preserves and respects the patient's natural facial features, restoring a refreshed and radiant look.

Revision Rhinoplasty for a young woman who had an unsuccessful primary rhinoplasty three years ago. She presented to our clinic with indentations in the alar region, a slightly bulbous nasal tip, a mildly deviated nasal dorsum, inferiorly retracted (or caudally drooped/hooded) nostrils, and a slightly ptotic overall nasal appearance. During the surgery, we utilized a costal cartilage graft and temporal fascia, with which we meticulously reshaped the nose. We, of course, addressed the nasal dorsum, the alae, and the nasal tip, and the harmonious and aesthetically pleasing result is presented before you.
Remember, a secondary/revision rhinoplasty following an unsuccessful primary nose job is a complex procedure that requires a high level of surgical expertise combined with extensive experience in revision nasal surgery cases.

שנה לאחר ניתוח אף אולטראסוני, בו בוצע תיקון של אף עקום עם סטייה משמעותית של ציר האף לימין. מדובר במקרה מורכב שמצריך מיומנות גבוהה של המנתח בביצוע יישור מדויק של האף. כמו כן, טיפלנו בגבנון הגדול ובטיפ הנפול ליצירת מראה גברי וטבעי.

One year after ultrasonic rhinoplasty, which involved crooked nose correction to address a significant deviation of the nasal axis to the right. This was a complex case requiring a high level of surgical expertise to achieve precise realignment of the nose. In addition, we refined the prominent dorsal hump and lifted the drooping tip to create a natural, masculine appearance.
