🚨 Emergency Splenectomy in a Young Trauma Patient

A young male presented after a high-speed road traffic accident with:
🩸 Grade 2 splenic injury
🩸 Mesenteric tear

Despite aggressive resuscitation and massive transfusion (6 units PRBCs), his hemoglobin continued to fall — a clear sign of ongoing internal bleeding.

🛑 No access to interventional radiology or splenic artery embolization.
🚨 Hemodynamic instability worsened.

✅ Emergency splenectomy was performed.
Intraoperative findings:
🔹 Gross hemoperitpneum with bleeding from splenic laceration
🔹 mesenteric tear with hematoma

🔍 Key Insight:
While Grade 2 spleen injuries are usually managed conservatively, surgery becomes life-saving when:
➡️ There’s ongoing blood loss
➡️ Patient is unstable
➡️ Non-operative options aren’t available

🧠 Always let clinical judgment and available resources guide trauma care — not just injury grading.
