Procedure Overview:
Exposure: The shoulder joint is exposed through an incision.
Humeral Head Removal: The damaged humeral head (ball fragment) is removed.
Socket Replacement: A metal hemisphere (artificial ball) is attached to the shoulder blade (glenoid).
Humeral Stem Insertion: A metal prosthesis with a plastic cup insert is implanted into the shaft of the humerus (upper arm bone).
Tuberosity Reconstruction: The tuberosities (bone fragments where tendons attach) are realigned and fixed to the prosthesis and humeral shaft with sutures or cables.
Bone Grafting: The fixation is often supplemented with bone graft from the removed humeral head fragment.
Benefits:
Improved Deltoid Function:
The reverse design allows the large deltoid muscle to become the primary mover for shoulder elevation, even with a torn rotator cuff.
Better Outcomes:
Studies have shown that reverse shoulder replacement can lead to better pain scores and functional outcomes compared to hemiarthroplasty for proximal humerus fractures.
Less Reliance on Tuberosity Healing:
Reverse shoulder arthroplasty has been shown to be less dependent on the healing of the greater tuberosity fragment for good functional outcomes.

An osteochondral fracture (OCF) is a fracture that involves both the cartilage and the underlying bone.
How does Patellar Dislocation Cause OCFs?
During a patellar dislocation, the patella (kneecap) slides out of its normal position, and this can cause damage to the articular cartilage and underlying bone, leading to OCFs. Importance of Early Intervention:
Prompt diagnosis and treatment of OCFs are crucial to prevent long-term complications, such as recurrent patellar dislocation, cartilage damage, and the development of osteoarthritis. Reverse Shoulder Replacement is a boon for elderly people with shoulder arthritis. Indications for Reverse Shoulder Replacement:
Rotator Cuff Tear Arthropathy: The most common reason for considering a reverse prosthesis is when there’s arthritis of the shoulder joint and the rotator cuff tendons are torn or gone.
Severe Glenoid Arthritis: Severe arthritis of the glenoid (shoulder socket) can also be a reason for considering a reverse shoulder replacement.
Failed Traditional Shoulder Replacement: If a standard shoulder replacement doesn’t provide adequate relief or function, a reverse replacement may be an option.
Fractures of the Proximal Humerus: In some cases of complex fractures of the upper arm bone (proximal humerus), a reverse shoulder replacement may be indicated.
Massive Rotator Cuff Tears: If the rotator cuff tendons are all torn and a person cannot lift the arm high enough to function, a reverse prosthesis could help regain motion and function.
