Happily discharged 2 *Organophosphate Poisoning* patients last week..

1st was "2.5 months" baby who was
referred to us with lethargy, secretions, limpness, hiccups and Jerky breathing with seizures since 5-6 hours.

Initially on admission we didn't thought of poisoning in such a small exclusively breast feed baby...
But on detailed clinical examination and with past experience, we got some clue and started treatment as per OP poisoning protocol!

Later reports came with serum cholinesterase levels abnormal

By the time reports came the baby was already settled with our timely treatment & early decisions and prevented her from ventilatory support and related complications...

The baby was improved by day 3 and discharged on day 5...

2nd case
was 8 months baby referred from remote place in ambulance in a very bad situation with respiratory failure with provisional diagnosis of ?Aspiration as the baby had short history of vomiting since afternoon

Thanks to the referring doctor who referred her timely and with proper ambulatory support as little delay in decision would have been life threatening...

On admission she was intubated and taken on ventilatory support ...
Detailed history was taken but didn't reveal any contact with any Poison as such..

But clinical examination was again more in favor of poisoning

Again cholinesterase reports were abnormal and early and proper decisions saved her life without any complications..

Smoothly came out of ventilatory support on day 3 and discharged on day 7 ...

Thanks to our teamwork and great efforts of our dedicated and trained Nursing and hospital staff !

These 2 cases again taught lesson and remembered our teachers advice of importance of clinical examination over anything else...
