Reviving this page with a new identity. Here's something interesting to begin- Guided Block harvest and positioning inspired by Luca De Stavola for a middle aged lady with Sjogren's syndrome.The turnover of both the autologous bone as well as the Porcine Carbonated Hydroxyapatite were excellent both radiographically and clinically, however, there was a deviation from the original plan at implant placement, with final position getting deeper than intended.

Bisphosphonate related osteonecrosis can rear its ugly head at any time.. this patient had her 37,38 extracted in Sep 2019 and had seemingly normal healing til she showed up in June 2020 with ONJ of the left mandible.. three surgical attempts by 3 different surgeons later, she hasn’t got coverage of the bone on that side. The two implants on the right were loaded in Dec 2018, and were absolutely fine till a month ago.. she showed up 4 days ago with exposed dead bone in what was otherwise a healthy looking site. You can see the extent of damage on the left side in the last 3 years and well as the destruction on the right. The last molar on the right and the maxillary have needed treatment for a couple of years but have been postponed repeatedly due to various reasons: medical and personal. She has been a chronic RA patient on steroids, methotrexate, HCQS, and lately Oral Idrofos (Ibandronate) since 2020.

The great benefit of being invited to speak at conferences is the opportunity to meet friends..the banter, laughter, camaraderie : priceless. The recently concluded Hard and Soft tissue summit of the ISOI in Mumbai was quite a scientific and social treat.. high quality lectures by Salah,Komal,Ali, Tarun, Rita, Ashvini, Teja. Udatta, Pinak to name a few, and a captive audience over the two days, all the way to the last lecture: kudos to the organising team for a fantastic show..

Maxillary lateral incisor socket shield done in Aug ‘21, provisional using his old EMAX crown.patient had a MI in December and didn’t come back for final restoration.
I saw him last week to make an intraoral scan , the images showing the healed site are from that appointment . Final restoration was delivered by a colleague , and I don’t have a clinical image, but post restoration CBCT shows stable bone around the implant and a solid, stable shield.
