Case Summary:
A 75-year-old female presented with a severely varus osteoarthritic knee, gross restriction of movement, and a proximal tibial stress fracture, resulting in inability to walk. She had undergone total knee replacement (TKR) on the contralateral side at Apollo Hospital 7 years earlier.

Given the degree of deformity and instability, a Smith & Nephew semi-constrained implant was chosen to manage soft-tissue laxity. A tibial stem extension was additionally used to bypass and stabilize the proximal tibial stress fracture.

Postoperative Outcome:
Surgery resulted in restoration of mechanical alignment with stable fixation. The patient achieved a corrected limb axis with excellent intraoperative stability.
