Affection of gleno-humeral joint, characterised by pain and limitation of shoulder movements which slowly recover over a period of several months.

*CLINICAL FEATURES*

*Pterygium*

It is a triangular fold (tissue growth) of conjunctiva encroaching the cornea in the horizontal meridian, in the palpebral fissure.

- It is usually pinkish light yellowish in color.

*Etiology -*

- The exact cause is not clear.

- It usually occurs in elderly males doing outdoor work (long term exposure to UV light, dust, wind or sand is the preceding factor).

- Males are affected more than females as the ratio of 2:1

*Types -*

- It may be Unilateral or bilateral.

- It may start from nasal side i.e. "Nasal pterygium"
Or may also occur on the temporal side of the Bulbar conjunctiva i.e. "Temporal pterygium"
Or from both the sides.

*Parts of pterygium -*

It consists of -
~ Head
~ Neck
~ Body

*Stages of pterygium -*

a) Progressive stage -

~ Pterygium is thick, fleshy & vascular.
~ There is opaque spots in the cornea, in front of the head.
~ It progresses in the cornea, Bowman's capsule is destroyed.

b) Atrophic stage -

~ Pterygium is thin, attenuated with very little vascularity.
~ No opaque spots in the cornea.

*Symptoms -*

- Foreign body sensation.

- Watering from the eyes.

- Itching of eyes.

- Usually no vision related symptoms unless the Pterygium encroaches the pupillary area, once it reach pupillary area it affects the vision.

- Occasionally there is "Diplopia", due to limitation of movement of the eyeball.

- Many patients complaints regarding cosmetic appearance of the eye.

*Pathology -*

- Pterygium is characterised by vegetative changes of the subconjunctival tissue i.e. elastotic degeneration of collagen (actinic elastosis) and fibrovascular proliferation.

*Diagnosis -*

- Pterygium can be diagnosed without need for a specific examination,
But corneal topography is the test where the progress can be determined.

*Treatment -*

- Pterygium typically do not require surgery unless it causes visual problems.

- For irritation artificial tear drops are advised

*Surgical treatment -*

Pterygium excission followed by corneal auto grafting is the procedure.

For example, your genetic makeup may keep cells from removing LDL cholesterol from your blood efficiently or cause your liver to produce too much cholesterol.

RISK FACTORS--Factors that may increase your risk of high cholesterol include:

Heart attack. If plaques tear or rupture, a blood clot may form at the plaque-rupture site — blocking the flow of blood or breaking free and plugging an artery downstream. If blood flow to part of your heart stops, you'll have a heart attack.
