A 70 old male with history of cva presented with altered sensorium breathing difficulty fever and cough. His intial xray s/o complete collapse of right lung and abg showing low pao2 s/o of type 1 failure. Emergency bed site bronchoscopy was performed for relieving any endobronchial obstruction ...bal specimen was also taken for culture guided treatment.
Post bronchoscopy xray complete aeration of right lung restored. BAl culture showed e coli with sensitivity guided treatment pt was discharge with sensorium and breathing normalized.
Teaching point
1) old age and cva are risk factor for aspiration pneumonia as both causes impaired cough reflex
2) some time thick secretion and mucous plugs can cause collapse of lungs and respiratory failure
Bronchoscopy can help in both with removing mucous/secretion improving ventilation and also provide specimen for culture guided treatment for prompt recovery
