Brain is surrounded by a fluid known as Cerebrospinal fluid (CSF) which is commonly known as 'Brain fluid'. It gives nutrient to brain and it works as a shock absorber. Sometimes due to various reasons it can leak inside the nose through a defect in skull base bone. Bacteria can enter inside the skull through this bone defect and cause meningitis. Traditionally, this defect was closed by opening the skull. Now, the defect can be closed endoscopically through the nose without opening the skull.

Tumours of the nose and sinuses reaching the skull base needed an open approach by opening the skull and retracting the uninvolved brain.
Currently, some of these tumours can be resected endoscopically through the nose without opening the skull(craniotomy) and without compromising tumour clearance. This type of surgery is known as Endoscopic Craniofacial Resection. Problems associated with brain retraction in open surgery can be avoided. All tumours are not suitable for this method. Case selection is very important to get good result.
