Hysterectomy में Ovaries निकाली जाती है या नहीं? | Total Laparoscopic Hysterectomy

आज हमने एक 50 साल की patient का Total Laparoscopic Hysterectomy किया, जिसमें uterus और fallopian tubes निकाले गए लेकिन ovaries को preserve किया गया।

Hysterectomy से पहले ज्यादातर महिलाओं का यही सवाल होता है कि क्या मेरी ovaries भी निकाली जाएंगी, और क्या इससे जल्दी aging शुरू हो जाएगी या bones और heart पर असर पड़ेगा।

इसका एक clear medical rule होता है। अगर patient की age 50 साल से कम है और ovaries healthy हैं, तो हम ovaries को preserve करते हैं। Ovaries को बचाने से body में natural hormones बनते रहते हैं, जो bones, heart और overall health के लिए जरूरी होते हैं।

लेकिन fallopian tubes को आमतौर पर remove किया जाता है, क्योंकि कई बार cancer की शुरुआत tubes से हो सकती है और age बढ़ने के साथ risk भी बढ़ता है।

अगर age 50 साल से ज्यादा है, तो uterus के साथ ovaries और tubes को भी remove करने की सलाह दी जा सकती है, यह पूरी तरह patient की condition और risk factors पर depend करता है।

35 Weeks में Baby सिर्फ 1.8 Kg? Low Birth Weight Pregnancy में क्या करें!

अगर pregnancy में baby का weight जितना होना चाहिए उतना नहीं बढ़ रहा है, तो घबराने की जरूरत नहीं है। लेकिन extra ध्यान रखना बहुत जरूरी होता है।

ऐसे cases में हम Doppler ultrasound और regular monitoring से baby की condition पर नजर रखते हैं, ताकि सब कुछ safe रहे।

अभी pregnancy 35 weeks 3 days की है और baby का estimated weight लगभग 1.8 kg है। Ultrasound का weight 100 percent exact नहीं होता, इसमें थोड़ा फर्क हो सकता है।

इस stage पर तुरंत delivery कर देने से baby को NICU में ज्यादा दिन रहना पड़ सकता है। क्योंकि lungs और बाकी organs को पूरी तरह mature होने के लिए थोड़ा और समय चाहिए।

इसीलिए steroid injection दिया जाता है ताकि अगर early delivery की जरूरत पड़े तो baby के lungs ज्यादा safe रहें।

Hospital में 24 by 7 doctors, pediatric care और in house NICU available है। इसलिए किसी भी emergency में तुरंत decision लिया जा सकता है।

अगर monitoring में सब कुछ normal रहता है, तो 37 weeks पर planned delivery baby के लिए ज्यादा safe रहती है।

Newborn Jaundice in First 24 Hours: क्या यह Normal है?

Birth के 24 hours के अंदर baby का bilirubin level 8.4 mg per dL पाया गया, जबकि सामान्य cut off लगभग 7.5 mg per dL माना जाता है।

इसका मुख्य कारण ABO incompatibility है, क्योंकि mother का blood group O positive है और baby का blood group B positive है।

ऐसी स्थिति में mother के anti A और anti B antibodies baby के blood में जाकर हल्का hemolysis कर सकते हैं, जिससे early neonatal jaundice और bilirubin level बढ़ जाता है।

Baby को single surface phototherapy दी जा रही है, जो newborn jaundice में एक सुरक्षित और effective treatment है जब bilirubin treatment limit से ऊपर चला जाता है।

Phototherapy के दौरान baby की eyes को सुरक्षित रूप से cover किया जाता है।

समय पर treatment मिलने से ज्यादातर cases में bilirubin 24 hours के अंदर control हो जाता है और आगे कोई गंभीर समस्या नहीं होती।

Newborn jaundice में सही समय पर जांच और इलाज बहुत जरूरी है ताकि complications से बचा जा सके।

True Labor vs False Labor Pain:कैसे पहचाने अस्पताल जाने का सही समय! |

डिलीवरी से पहले लेबर पेन को समझना बहुत जरूरी है ताकि सही समय पर अस्पताल पहुंचा जा सके। लेबर पेन दो प्रकार के होते हैं। एक False labor और दूसरा True labor।

False labor को Braxton Hicks भी कहा जाता है। यह दर्द ज्यादातर uterus के stretch होने की वजह से होता है। खासकर twin pregnancy में uterus ज्यादा फैलता है, इसलिए इस तरह का दर्द महसूस हो सकता है।

यह दर्द नियमित नहीं होता और समय के साथ बढ़ता नहीं है।

True labor में cervix खुलना शुरू होता है, जो डिलीवरी की तैयारी का संकेत है। इस दर्द की खास बात यह है कि यह धीरे धीरे बढ़ता है।

शुरुआत में दर्द एक घंटे के अंतर से हो सकता है, फिर हर तीस मिनट में, फिर पंद्रह मिनट में और बाद में हर पांच मिनट में आने लगता है। जब contractions नियमित और तेज हो जाएं, तो तुरंत डॉक्टर से संपर्क करें और अस्पताल पहुंचें।

अगर तय की गई डिलीवरी डेट से पहले कोई भी लक्षण महसूस हों तो इंतजार न करें। समय पर जांच मां और बच्चे दोनों की सुरक्षा के लिए जरूरी है।

36 Weeks Pregnancy में Cesarean Section | High Risk Pregnancy Management

आज हमने 36 weeks की pregnancy में Gravida 3 मरीज पर एक सफल Cesarean Section किया।

मरीज की previous 2 abortions की history थी और baby में IUGR (Intrauterine Growth Restriction) था।

Dopplers डिरेंज्ड थे और मदर का Blood Pressure लगातार high था, जिसे हमने careful monitoring के साथ manage किया।

Diastolic flow में बदलाव और continuous hypertension को देखते हुए हमने Cesarean Section जल्दी करने का निर्णय लिया ताकि मदर और baby दोनों सुरक्षित रहें।

Procedure के दौरान खास ध्यान Bladder dissection पर दिया गया ताकि कोई injury ना हो।

Baby girl weighing 1.78 kg सुरक्षित रूप से delivered हुई।

Baby को थोड़ी mild respiratory distress थी, जो low birth weight की वजह से expected थी और proper care के साथ resolve हो जाएगी।

Baby अभी stable है और oxygen saturation, heart rate और blood sugar monitoring के तहत है।

इस case से पता चलता है कि high-risk pregnancy में timely intervention, careful surgical technique और expert monitoring कितनी महत्वपूर्ण है।

4 Kg का Baby Cesarean Delivery से: High Birth Weight Baby Delivery Complications!

हाल ही में हमने एक 4 kg baby की cesarean delivery की।

जब baby का weight ज्यादा होता है, तो mother में Postpartum hemorrhage का risk अपने आप बढ़ जाता है।

इसका कारण यह होता है कि uterus ज्यादा फैल जाता है और delivery के बाद उसका tone कमजोर हो सकता है, जिससे bleeding का खतरा बढ़ जाता है।

ऐसे cases में risk को पहले से anticipate करना बहुत जरूरी होता है।

Time पर uterotonic medicines देने से uterus अच्छी तरह contract करता है और bleeding को control किया जा सकता है।
