Post-tonsillectomy bleeding management:
post-tonsillectomy hemorrhage is a surgical emergency because it can lead to airway obstruction or hypovolemic shock. Management focuses on airway protection, resuscitation, and urgent ENT intervention.

Post-tonsillectomy bleeding management
1. Immediate assessment (ABC approach)

A.Airway
-Sit patient upright and leaning forward to prevent aspiration.
-Suction blood/clots from mouth.
-Prepare for intubation if airway compromise.

B.Breathing
-Give high-flow oxygen.

C. Circulation
-Establish two large-bore IV lines.
-Monitor BP, pulse, SpO₂.
-Send blood for:
CBC
Coagulation profile
Cross-match
-Post-tonsillectomy hemorrhage should always be treated seriously even if bleeding appears small.

2. Initial resuscitation
-IV fluids (crystalloids if hypovolemic)
-Blood transfusion if significant blood loss
-Keep patient NPO
-Continuous monitoring

3. Temporizing measures to control bleeding
These help stabilize the patient until definitive surgical control.
Local measures
-Gentle suction of clots
-Apply cold saline gargle or ice water gargle
-Direct pressure with adrenaline-soaked gauze (in cooperative patient)

Medications
-Tranexamic acid (TXA)
IV TXA
Nebulized TXA (in some protocols)
Consider topical vasoconstrictors (adrenaline)
TXA may help stabilize the clot and reduce bleeding temporarily.

4.Hospital admission
-Even if bleeding stops → admit for observation 12–24 hours.

5. Definitive management
-If bleeding continues or recurs:
-Return to operating theatre
-Examination under anesthesia
-Cauterization or ligation of bleeding vessel
-Suction diathermy / suture ligation

6. Red-flag signs
-Active bleeding
-Repeated vomiting of blood
-Tachycardia / hypotension
-Excessive swallowing in children
-Blood on pillow
These require immediate emergency management.
