Maggots debridement therapy:
“Maggots debridement therapy” also known as larva therapy, bio debridement or bio surgery are practiced widely in Western countries. It manages to clean the wound by removing dead and infected tissues disinfecting the wound (kill bacteria).
The therapy is a debridement method where the larva of the LUCILIA SERICATA FLY are applied on the wound ,the larva ingest and break down dead tissue and also decrease the bacterial load on the wound was first observed and inculcated in early 1930s and soon in 1940’s popularity of Maggots debridement therapy was declined due to introduction of antibiotics and again in late 1980’s it gained back its popularity due to increase in incidence of pressure ulcer, leg ulcer and diabetic foot ulcer.
The “Maggot therapy” has been explained in our classical treaties “Sushrutha Samhita” by the father of indian surgery “Acharya Sushruta” in its extent form in 184 chapters contains description of 1120 illnesses. “Sushrutha Samhita” chikitsa sthana ,chapter 18 in the context of Arbuda, sloka 37 to 40 as mentioned “Krimi Utpattikara Chikitsa” as Maggot therapy.
The technique became widely used in north america and was extended to the treatment of other lesions where there was pyogenic infection or tissue necrosis.
The available reference says that this procedure was started during the late 1920s by “William bear” at John Hopkins University in Baltimore Maryland,He first studied lifecycle of maggots successfully, treated the wounds and published a series of article and he recommended to rear and disinfect the maggots and he is the one to recommend application of specific species of blowflies.
History:
Dr. Joseph Jones, a ranking Confederate medical officer during the American Civil War, is quoted as follows, "I have frequently seen neglected wounds ... filled with maggots ... as far as my experience extends, these worms eat only dead tissues, and do not injure specifically the well parts." The first therapeutic use of maggots is credited to a second Confederate medical officer Dr. J.F. Zacharias, who reported during the American Civil War that, "Maggots ... in a single day would clean a wound much better than any agents we had at our command ... I am sure I saved many lives by their use." He recorded a high survival rate in patients he treated with maggots.
Selection criteria:
This systematic review included, studies in English,randomised control trial, clinical control trials, comparative studies, retrospective studies, prospective studies and systematic reviews, the chronic wounds included in the review were pressure ulcer, diabetic foot ulcer and leg ulcer.
Conclusion:
For maggots therapy the Debridement of necrotic tissue with the sterile fly larve is fast securing a place in wound care krimi utpattikara chikista which is explained in our classics ,which is an effective and safe treatment option for debridement and disinfection.
