Spine is like central pillar or main trunk of our body. Spine has few basic duties. It transfer upper body parts weight to lower limbs/legs, it protects the spinal cord. It gives flexibility as well as stability to our body. Normally this machine functions very well to accomplish its responsibilities. But sometimes this machine malfunctions due to various reasons.
Common spine problems, symptoms and treatment:
1. Sciatica/ slipped disc – young to middle aged active individual presents with acute/subacute back pain with severe one leg pain making walking standing almost impossible or very painful. MRI proves the diagnosis. Treatment depends upon the severity of slip disc – mild cases require bed rest for 6 weeks with pain control. Moderate cases can be treated with spinal block injections. Severe cases are best treated with micro surgery.
2. Spondylosis/ age related back pain - with age our spine starts to malfunction if it is not properly taken care of. The discs In between vertebra’s tends to worn out after the age of 45 – 50. That gives rise to back pain, decreased endurance, easy fatiguability. Difficulty in getting up, bending, sitting in the ground, long travel. Treatment is mostly symptomatic - exercise, weight reduction, yoga, core strengthening exercise, and short term pain management.
3. Lumbar canal stenosis – when spondylosis advances it gradually reduces space for spinal nerve. Nerve adjusts in a narrow space till its no longer could manage. After critical narrowing walking becomes slow and difficult due to nerve pain in the legs. Patient complains heaviness, pain, tingling, numbness in his legs while walking. If left untreated this condition may give rise to permanent nerve damage or partial or complete paralysis of nerves. Most of the tie thos advanced problem best treated with surgical management. Which Is a very safe and effective option. The space for the nerves are recreated and the leg pain disappears.
4. Spondylolysis; young individuals sometimes have congenital defect of vertebra which gives rise to early onset back pain with sporting activity. This condition is usually treatable with rest and medicines.
5. Spine fracture: any high velocity fall or road traffic accident can give rise to spine fractures. It’s a dangerous condition as there may be injury to spinal cord which gives rise to partial and complete permanent paralysis. For this grave injury early surgery by an experienced spine surgeon can save patient from permanent paralysis. It is usually happens to young active working class so the whole family’s future depends upon the wellbeing of injured spine patient
6. Osteoporotic spine fracture: this is completely different from acute fracture. Osteoporosis is a disease where the bones in whole body becomes weak due to deficiency of calcium. The bones may become so weak that it gets fractured by own body weight or trivial trauma. Usually it affects very old(65- 80years) age group. Patients complains of moderate to severe pain in the back when getting u or turning in bed, with time it aggravates. Treatment is bedrest with calcium, if not getting better a small daycare procedure called vertebroplasty where bone cement is put to fill the bony gap can reduce the pain immediately.
7. Spine infection: it can be bacterial or TB. If any back or neck pain which is continuous, increases with bed rest, associated with fever, weight loss and not relieved by pain killer , the chance of infection is there. It is proven by MRI, treatment is antibiotic with or without surgery which your spine specialist doctor will decide.
8. Scoliosis: where the spine has a sideways curve from very young age group(5- 15 years). The curve can be corrected by surgery.
9. Congenital: birth defects of spine which your spine doctor will evaluate and tell you the correct treatment.
