🔹 Serum Creatinine: 3.6 mg/dL
🔹 Urine: 2+ protein, no RBCs
🔹 Urine PCR: 0.9
🔹 Ultrasound (KUB): Bilateral small kidneys, ECHO - Conc LVH with Normal LV.

👉 Diagnosis: Presumed Chronic Kidney Disease secondary to long-standing uncontrolled hypertension

He was initially on multiple antihypertensives:
• Nicardia 20 mg TID
• Minipress 5 mg TID
• Moxonidine TID
• Cilacar 10 mg BD
• Met XL 50 mg BD
