Hijama (Cupping therapy) in Islam:

Prophet Muhammadﷺ Said: "The best medicines you may treat yourselves with are cupping." (Sahih Bukhari:5696)

Prophet Muhammadﷺ Said:
"The best (treatment) which you take is cupping, or it is the best of your treatments. [Sahih Muslim: 1577 (4038)]

Cupping therapy is one of the oldest and most effective method of releasing the toxins from body tissue and organs. It is also known as vacuum cupping, hijama cupping, horn treatment etc. It is a practice in which the therapist puts special cups on the skin to create suction. This causes the tissue beneath the cup to be drawn up and swell causing increase in blood flow to affected area. Enhanced blood flow under the cups draws impurities and toxins away from the nearby tissues and organs towards the surface for elimination.

Cupping is the Best of Remedies

“If there is something excellent to be used as a remedy, then it is Hijama Cupping”.
Reported by Abu Huraira.[Sunan Ibn Maajah]

The Prophet (PBUH) explained that Hijama is one of the best medicines: “The best medicine with which you treat yourselves is Hijama, or it is one of the best of your medicines,” or “The best treatment you can use is Hijama.” [Sahih Bukhari Hadith-No: 5371]

In Hijama Cupping there is a Cure

The Prophet (PBUH) also said: “If there is anything good in the medicines with which you treat yourselves, it is in the incision of the Hijama therapist, or a drink of honey or cauterization with fire, but I do not like to be cauterized.” [Sahih Muslim, Hadith No: 2205]

Jaabir ibn Abdullah y reported that the Messenger (PBUH) said, “Indeed in cupping (hijama) there is a cure.” [Saheeh Muslim hadith No: 5706].

Best Days for Hijama

Abu Hurairah(r.a) reported that the Messenger (PBUH) said, “Whoever performs cupping (hijama) on the 17th, 19th or 21st day (of the Islamic, lunar month) then it is a cure for every disease.” [Sunan Abu Dawud Hadith No: 3861].

Abdullah ibn Abbas reported that the Messenger (SAW) was cupped on his head for
a unilateral headache while he was in Ihraam. [Sahih al-Bukhari (5701)].

Ibn ‘Abbaas (may Allaah be pleased with them both) reported that the Prophet (PBUH) was treated with cupping, and he paid the cupper his fee.” (Reported by al-Bukhaari, 10/124; Muslim 1202).

Cupping for Women

Jaabir ibn Abdullah (RA) reported Umm Salama (RA) asked permission from the Messenger (PBUH) to do cupping. So the Messenger (PBUH) ordered Abu Teeba (RA) to cup her. Jaabir ibn Abdullah (RA) said, ” I think he (SAWS) said that Abu Teeba (may Allaah be pleased with him) is her brother through breastfeeding or a young boy who didn’t reach puberty”. [Saheeh Muslim - Hadith No: 5708, Sunnan Abu Dawud - Hadith No: 4102), Sunan-Ibn-Maajah Hadith No: 3480].

The above quoted Hadith is clear that cupping was practiced by The Prophet (PBUH) himself and strongly recommended by him.

Jaabir ibn Abdullah Razi Allahu anhu reported that the Messenger (PBUH) fell from his horse onto the trunk of a palm tree and dislocated his foot. Waki’ said, “Meaning the Messenger (PBUH) was cupped on (his foot) for bruising.” [Saheeh Sunan ibn Maajah (2807)].

PRECAUTIONS AND CONTRAINDICATIONS:

Cupping therapy, in general, is very safe and has no side-effects. There are a few exceptions, however, and when these are observed the implementation is safe and the benefits are generous. Precaution is necessary when treating patients who may be on anticoagulant drugs, or have an empty stomach, particularly during long fasting periods such as the holy Ramadan month, as fasting temporarily weakens the body’s energy.

Do not employ cupping therapy over the eyes, sunburn or burns in general, open wounds or a recent trauma. During pregnancy, avoid cupping to the lower and upper abdomen; the lower back can be cupped until the sixth month of pregnancy, using Light or Medium cupping methods only (see Plates 33 and 34 in the colour plate section).

In patients complaining of lethargy or exhaustion, Empty cupping should be employed for only a very short time (i.e. a maximum of 5 minutes). During the Bleeding cupping application, patients on anticoagulant drugs may bleed more than originally calculated. It is therefore necessary to monitor the bleeding and remove the cup when about 100 mL of blood has been extracted. Such patients also mark or bruise more quickly and the skin takes longer to recover to its normal colour. Cupping (Wet or Dry) is contraindicated for extreme Yin-Xu (fluid deficient) people, in people suspected of haemorrhage of any kind, and on tumours of any form, including tuberculosis. Also, cupping of any kind is contraindicated for people who have suffered a cardiac arrest in the last 6 months. Wet cupping is contraindicated for people suffering from haemophilia or extreme anaemia. Avoid Wet cupping over the large veins such as varicose veins. Cupping therapy is contraindicated in all stages of an acute infectious conditions and diseases.

Cupping therapy is a simple, effective and economic treatment. Cupping therapy is a formal treatment in Chinese hospitals and in some European countries. Here, we review history of wet cupping therapy and its two distinct methodologies. First method is puncturing and cupping (PC) method that includes five steps: skin demarcation, sterilization, puncturing, cupping and sterilization. Second method is cupping, puncturing and cupping (CPC) method that includes six steps: skin demarcation, sterilization, first cupping, puncturing, second cupping and sterilization. CPC method predominates in the Arabic world, while PC method has a worldwide distribution. We report here that our detailed evidence-based analysis of both methods reveals many novel advantages in the CPC method over the PC method. CPC method has a better analgesic effect and benefits more from the viscoelastic nature of the skin. During the first cupping step in the CPC method (not present in the PC method), a fluid mixture containing causative pathological substances (CPS) is collected inside skin uplifting due to the effect of negative pressure of sucking cups. CPS differs from disease to disease according to disease etiology and pathogenesis. That fluid mixture contains collected interstitial fluids with CPS, filtered fluids (from blood capillaries) with CPS, hemolyzed blood cells (hemolysed RBCs, WBCs and platelets). That fluid mixture does not contain intact blood cells as their sizes are much bigger than sizes of pores of skin capillaries and cannot be filtered. The fluid mixture is totally excreted in next steps. We review here that wet cupping therapy is a highly recommended treatment in prophetic medicine, medicine related to Prophet Mohammad peace be upon him who said: “The best among what you use in therapy is Al-hijamah (CPC method of wet cupping therapy)”. In conclusion, CPC method is better than PC method in clearing blood and interstitial spaces from CPS. Standardization of methodologies of cupping therapy seems essential to gain optimal therapeutic outcomes.
