Poorly differentiated SCCa in 72 yr old female doctor with tumor extending intracranial with brain parenchymal invasion. Endoscopic craniofacial resection done with resection of brain parenchymal part of tumor through the nose. Image guidance was used .Pt had CSF leak on day 6 which was closed with pericranial flap tunneled through a 1cm hole in the anterior wall of the frontal sinus obviating the need for craniotomy. The patient also received post operative adjuvant chemo RT.
