Integrative Medicine Approach

A**l fissure is a superficial tear at the A**s.

CAUSES:-
Local trauma due to hard stools
Inflammatory bowel disease.
A**l trauma
SYMPTOMS:-

1. Pain during passage of stools.
2. Spasm of the a**s while passing stool
3. Bright-red blood covers the surface of stools, in a small amount.

TREATMENT

ALLOPATHY
Topical anesthetic may be applied at the a**l area to relieve pain.
Diet modification to soften stools. (Stool softener)
Increase fruits, vegetables, and soluble and insoluble fibers in diet. (High-fiber diet)
Increase fluid intake (as needed)
Bulking agents such as psyllium
Warm water sitz bath after passing stools
Pain killers
Topical nitrates,
Calcium channel blockers,
Surgical therapy is reserved when conservative treatment fails to heal a**l fissure.

Dysmenorrhea means painful menstruation. It is of 2 types, Primary dysmenorrhea and secondary dysmenorrhea.

Causes:-
Primary dysmenorrhea occurs due to
1. prostaglandin F2α (PGF2α), a potent myometrial stimulant and vasoconstrictor, in the endometrium.
2. Early age at menarche (< 12 years)
3. Nulliparity
4. Heavy or prolonged menstrual flow
5. Smoking
6. Family history
7. Obesity

Secondary dysmenorrhea occurs due to :
1. Leiomyomata (fibroids)
2. Pelvic Inflammatory Disease
3. Tubo-ovarian abscess
4. Ovarian torsion
5. Endometriosis

Management
ALLOPATHY TREATMENT
For Primary dysmenorrhea
1. Medication are primarily used for relieving the pain of dysmenorrhea.
2. Pain killers
3. Combination OCs (eg, ethinyl estradiol with progestin or drospirenone)
4. Levonorgestrel intrauterine device
5. Depot medroxyprogesterone acetate

For Secondary dysmenorrhea
1. Correction of the underlying organic cause.

CAUSES

Movement of gastric secretions or bile from the duodenum and stomach into the esophagus is the cause of GERD.
Relaxation of the LES (Lower Esophageal Sphincter) is the most common cause of GERD.

LES relaxation can occur because of – Alcohol, coffee, chocolate, fatty food, medications like beta-agonists, nitrates, calcium channel blockers, anticholinergics, hormones like progesterone, and ni****ne.
