This patient is living with Post Polio Residual Paralysis (PPRP), which resulted in a significant leg length discrepancy (LLD). Due to the imbalance in limb length and altered alignment, walking independently became difficult and required continuous support. Charcot foot is a serious condition that mainly affects people with long-standing diabetes or nerve damage, where loss of sensation in the foot leads to repeated unnoticed injuries. Over time, these injuries cause weakening of bones and joints, resulting in swelling, instability, and a gradual change in the shape of the foot. If not managed at the right time, Charcot foot can lead to severe deformities and difficulty in walking.

When this patient noticed a change in her foot shape and increasing discomfort, she came to Carevale Rehab to prevent further deformity and protect her mobility. Meet Hasim, one of our young and determined below-knee prosthetic user from Somalia. He lost his leg in a road traffic accident and was previously using an old, ill-fitting prosthesis that caused discomfort and made walking difficult. This baby is diagnosed with CTEV (Congenital Talipes Equino Varus), commonly known as clubfoot, where the foot turns inward and downward due to abnormal positioning of bones and tightness of muscles and tendons. Because of this condition, the baby finds it difficult to maintain a normal foot position, which can affect standing, walking, and overall balance in the future.

After proper assessment and evaluation at Carevale, we provided him a Denis Brown Splint, which is a specially designed abduction splint used to maintain the corrected position of both feet. Its function is to keep the feet in proper alignment, prevent relapse of deformity, support muscle stretching, and guide the baby’s feet towards a normal walking pattern. The splint has a bar with two shoes attached at a specific angle to hold the position effectively.

She is one of our patient who has Cerebral Palsy, which caused excessive knee flexion and ankle dorsiflexion during walking. Because of this abnormal gait pattern, she was unable to walk efficiently and experienced pain during ambulation due to poor joint alignment and increased stress on the lower limb joints. This condition affected her stability, posture, and overall walking ability.

After a thorough clinical assessment and detailed gait analysis, we provided her with a customized 3D Floor Reaction Orthosis (FRO). The primary function of a Floor Reaction Orthosis is to control excessive ankle dorsiflexion and utilize ground reaction forces to promote knee extension during the stance phase of gait. By stabilizing the ankle and directing the ground reaction force anterior to the knee, the orthosis helps reduce excessive knee flexion, improves lower limb alignment, and enhances stability. It also reduces abnormal joint loading, decreases muscle fatigue, and supports a more efficient and balanced walking pattern.

After wearing the 3D Floor Reaction Orthosis, the patient experienced a significant improvement in her walking pattern. Her knee stability improved, ankle control became better, and overall posture during walking was more upright. She also reported reduced pain and increased confidence while walking, highlighting the effectiveness of the Floor Reaction Orthosis in improving functional mobility and comfort.

Carevale has recently adopted Direct Socket Design technology, an advanced and innovative approach in prosthetic socket fabrication that significantly reduces waiting time while enhancing precision and comfort. With this technology, the prosthetic socket is created directly on the amputee’s residual limb, ensuring an accurate, customized fit that closely matches the individual’s anatomy.

The entire socket fabrication process is completed within approximately 15 minutes, eliminating the need for traditional multi-step casting and prolonged trial sessions. This direct fabrication method improves stump-to-socket clearance, enhances weight distribution, and provides better suspension and comfort for the user. As a result, the prosthesis offers improved stability, confidence, and functional performance.

Remarkably, after just 15 minutes, the user can begin walking with the prosthesis, making this technology a major advancement in delivering faster, more efficient, and patient-centered prosthetic care at Carevale.
