Transhiatal esophagectomy is a surgical procedure used primarily for removing the esophagus in cases of esophageal cancer, strictures, or achalasia, performed without opening the chest cavity. The procedure involves incisions in both the abdomen and the neck, with the stomach mobilized and pulled up to replace the resected esophagus, typically creating a cervical esophagogastric anastomosis.

**Distal Ileal Resection + Anastomosis for Distal Ileal Stricture Presenting as Intestinal Obstruction**

Today we managed a case of intestinal obstruction caused by a distal ileal stricture. The patient underwent a successful distal ileal resection followed by end-to-end anastomosis. The resected specimen clearly shows the narrowed segment responsible for the obstruction.

Distal ileal strictures may present with features of small bowel obstruction, and prompt surgical intervention is essential to prevent complications. Postoperative recovery is expected to be uneventful.
