You guys made it possible to perform two different microsurgery and endovascular case on same day. *It’s true form of Holistic care for cerebrovascular diseases*. Despite patients arriving to us in critical state, we are able to deliver optimum- *THE BEST POSSIBLE.*

Endovascular case- Ruptured fusiform dysplastic right vertebral artery aneurysm managed with *Flow diverter stenting Pipeline Shield Flex*. Perfect positioning is important to spare opposite vertebral artery and critical perforators at vertebra-basilar junction medullary perforators.
Patient had uneventful hospital stay.

Microsurgical clipping case is very complex scenario but we delivered good result for patient. It will be shared in next case with surgical video.

3 T MRI visualising flow voids in pituitary Macroadenoma. Hypophyseal artery(supplying pituitary gland/tumor) and McConnel Capsular artery (Osteo dural artery 1953).

These flow voids size changes with size of tumor until they get compressed by tumor large size of tumor. In smaller size of tumor, traction on these direct branches while
Tumor decompression might lead to tear in ICA. It might bleed immediately or can form pseudo aneurysm.

In this case, we were well aware of these flow voids. Four handed approach, Early identification, bipolar coagulation and sharp dissection without traction helped in achieved safe resection of pituitary adenoma.

Patient had uneventful post operation course.

Happy New Year 2025!

The first week has been quite busy and exciting. I have joined Nims Heart and Brain Hospital - NHBH as Head of Neurosurgery Department. Its Unit of NIMS university dedicated to super speciality care. My primary goal will be to perform cutting edge neurosurgical work and to provide all established treatment with cost effectiveness to Indian population. CEREBROVASCULAR, SKULL BASE, COMPLEX Spine surgery, Neurotrauma with development of sub-speciality epilepsy and functional Neurosurgery will be my top priority for 2025.

We cannot predict future but thing under my control is to do my work with higher quality and I am dedicated to service of mankind forever.

EAR WAX:

Should you clean out your ears with cotton swabs? Is ear candling safe????

The dos and don’ts for ear care:
• Avoid overcleaning your ears, as this can lead to irritation in the ear canal, ear infection, and further earwax buildup.
• Do not put cotton swabs, hair pins, toothpicks, and other small items in your ears; doing so can damage the eardrum, ear canal, or even dislocate the hearing bones.
• Avoid using ear candles; they can damage the eardrum and ear canal, and there is insufficient evidence to suggest they are effective for removing excess earwax.
• Seek medical attention if you are experiencing ear pain, ear fullness, or hearing loss and you are unsure whether these symptoms are caused by earwax buildup.

I saw the angel and carved until I set him free. The Sculpture is already Complete within the marble block. I just have to chiesal away the unnecessary material. —— Michelangelo

Sixty five year old famous sculpture artist asked me first question after operation when he regained his counscious that Doctor -“I will be able to continue my profession or not?”

I just replied to him that I have done the same which you do to the marble block to carve out sculpture. I just removed the unnecessary.

After operation he is continuing his profession efficiently and made sculpture of Governor of Rajasthan Shri Kalraj Mishra.

Nasal septum is a straight wall made up of bone and cartilage which ideally divides nasal cavity in two equal parts.

In some cases, due to birth defect or any injury to nose, bends septum severely to one side .

Problems due to DNS:-
1)difficult to breathe through nose
2) chronic sinus infection,
3)nose bleed,
4)headache ,
5)snoring
6)cosmetic issues.
