The most commonly asked question by my pregnant mothers _

How do I plan for my delivery/admission to hospital with rising Covid cases?

1.Strict social distancing within 2 weeks from expected due date ,37 weeks in maximum patients.If feasible opt for work from home,avoid unnecessary travel.

2.All preoperative investigations, Covid testing preferably to be done a day before planned admission.

3.Only 1 attendant to accompany each patient,avoid attendant switching.Children and elderly strictly not allowed.

4.Wear surgical mask throughout your hospital stay.

5.Call early if complaints of reduced baby movements,pain abdomen,discharge or bleeding so that Covid testing is scheduled without delay with other treatment in time.

6.With rising numbers in young and children you need to be more vigilant.Other than the complications in the mother and the baby,difficulties during isolation and the discomfort of possibly not being able to deliver in the hospital of your choice are also reasons to worry.

How do I know about my risk of having Breast cancer?
Risk depends upon age,ethnic group(Asian),weight, height, age of 1st period,Age of stopping periods(if already menopause),age when 1st child was born,1 or more children.
Please see your doctor if more than 5 are yes.
1.Alcohol intake daily
2.Smoking
3.Breast cancer in family(mother,daughter,sister,aunt_mother or father's sister) esp. If less than 50 yes.age
5.You had uterine cancer
6.You had ovarian cancer
7.Do you have gene mutation linked to breast cancer.
8.Dense breasts on Mammogram/Taking COCs pills
9.You had radiation therapy to chest
10.History of intake of DES drug by your mother when pregnant.

If more than 45 yes and any risk factor,should consult your doctor

Some risk factors like age,family history and no full term preg cannot be changed.
But you can change others weight, smokingand alcohol.
Avoid hormones after menopause or restrict to lower doses /short periods.
Healthy diet and regular exercise

Having risk factor does not mean that you will develop Breast cancer.But having risk factors is a good reason to discuss with your doctor and schedule regular screening mammograms.

Screening advice for women at average risk
1.All women should know the benefits,/limits/potential harms of Breast Cancer Screening.
2.Should know about their breasts normal look and feel and report any changes.
3. 40_44 yrs=choice to start mammogram annually
45_54 yrs =mammogram yearly
More than 55 yes.=mammogram once every 2 years/continue yearly screening
Screening should continue as long as women are in good health or expected to live at least 10 more years

Physical activity in pregnancy _is it safe ,is it required?

Antenatal exercises
For most women it is important to do some regular physical activity as a part of healthy lifestyle. Moderate physical activity is safe in most cases. Min.of 30 min physical activity is recommended every day.
Walking is a very good physical activity.Exercises are usually started in the after the 1st 4 months of pregnancy.

It improves general well being,prevents low back pain,enhances physical and psychological preparation for delivery,increase joint stretching and muscle strength,reduces risk of clot formation in legs in pregnancy.

During exercising, breathe smoothly,work gradually as per your stamina.Do not overheat yourselves,take sips of fluids intermittently.
Stretching exercises,exercises for the back and pelvic floor exercises are recommended.Adopt proper posture.

Stop exercising if you have pain ,bleeding or any excess discomfort.Please consult your obstetrician for a detailed advice on these exercises.

FEVER IN PREG
Fever is when your oral temperature is >100.4 degree F

Fever is a sign that your body is fighting an infection. You should seek treatment. Do not ignore. Please take following precautions:
1.Take adequate rest.
2.Sweating can cause lack of water in the body so drink plenty of water.
3.Eat something, you require energy to recover from fever.
4.If temp.>100.4 degree F, see your doctor even without cold or flu like symptoms. Do not take medicines by yourself.
5.At the doctor’s clinic you may be advised some tests on your blood or urine sample to confirm cause. You can have vomiting or associated urinary complaints.
6.Fever is serious if-
your water bag around the baby is broken,
you have already started labour pains,
shortness of breath & chills, abdominal pain, neck stiffness.
7.Till you see a doctor, Paracetamol is the safest medicine for the shortest time.
8.NSAIDS (Ibuprofen, Aspirin) should be avoided while pregnant.
9.You can prevent yourself from falling ill by-
washing your hands often
stay away from sick people, crowded areas
avoid outside unhygienic food.
