Postural Changes in Low Back Pain and Neck Pain

1. Low Back Pain (LBP):
• Individuals with LBP often adopt protective or antalgic postures to minimize pain.

Common findings include:
• Flexed posture: Slight forward bending to reduce strain on painful structures.
• Lateral shift (list): The trunk may tilt to one side to alleviate nerve root compression.
• Flattened lumbar lordosis: Loss of normal curvature due to muscle spasm or guarding.
• Pelvic asymmetry: One hip may appear higher or rotated due to muscle imbalance.
• Prolonged poor posture (slouching, forward bending) can exacerbate pain by overloading discs and ligaments.

2. Neck Pain (Cervical Pain):
• Frequently associated with forward head posture (FHP) — the head projects forward beyond the body.
• Rounded shoulders and increased upper thoracic kyphosis often accompany it.
• Muscle changes include tightness in upper trapezius and levator scapulae, and weakness in deep neck flexors.
• Prolonged poor posture (e.g., screen use, phone use) increases cervical strain and triggers chronic myofascial pain.

Correction of Posture in Low Back Pain and Neck Pain

1. Low Back Pain

Goals: Reduce strain on spine, restore normal curves, strengthen core.
*Key Corrections*
• Maintain neutral spine: Keep the natural lumbar curve while sitting and standing.
• Ergonomic sitting:
• Sit with hips and knees at 90°.
• Use a small lumbar roll to support the lower back.
• Keep both feet flat on the floor.
• Avoid prolonged sitting: Take breaks every 30–40 minutes to stand or stretch.
• Standing posture: Distribute weight evenly on both feet, avoid leaning to one side.
• Sleeping posture: Prefer side-lying with knees slightly bent; use a firm mattress.
• Exercises: Core strengthening (planks, pelvic tilts) and stretching tight muscles (hamstrings, hip flexors).

2. Neck Pain

Goals: Restore head alignment, reduce muscle tension, improve ergonomics.
Key Corrections:
• Forward Head Posture Correction:
• Keep ears aligned over shoulders.
• Perform gentle chin tuck exercises several times daily.
• Workstation ergonomics:
• Screen at eye level; chair supports upper back.
• Avoid looking down at phones—raise device to eye level.
• Avoid prolonged static postures: Change positions every 30–45 minutes.
• Strengthening & Stretching:
• Strengthen deep neck flexors and scapular stabilizers.
• Stretch tight upper trapezius and levator scapulae muscles.
