Mechanical thrombectomy for acute ischemic stroke for tandem occlusion meaning occlusion at neck and brain vessels simultaneously. Large clot was removed from brain without cutting the skull and angioplasty was done for the narrowing of the carotid artery at neck. Patient got discharged and is improving neurologically. Carotid stenting is planned for a later date.

As an operator, I always believe in destiny. This 60y/f had rupture of left DACA aneurysm three times before reaching us. Patient was M3-M4 at presentation with grade 4 SAH, IVH with hydrocephalus, and right frontal hematoma. However, as she survived rupture of the anuerysm three times, I thought of taking a chance and do our best.

External ventricular drain was put, but not opened till the aneurysm was secured with coiling. Little bit neck left behind as I was not keen in putting any stent. After chemical angioplasty with intraarterial nimodipine and EVD opening, patient is showing signs of neurological recovery.

Mechanical thrombectomy for a 87 year old patient done using aspiration thrombectomy. He had a massive blockage of the right internal carotid artery and was treated by intravenous clot-bursting medicine (tenecteplase) followed by aspiration thrombectomy. Complete blood flow to the right side of the brain was restored and patient had a full neurological recovery.

The procedure was done at Manipal Hospitals,Mukundapur within the early window period of 6 hours thanks to the always ready stroke team.

Flow diverter assisted coiling of ruptured left ICA Aneursym. The aneurysm was secured without cutting the skull and through a pinhole in the groin.
