Hernia can be life threatening.
A 73 yrs man started episodic vomiting for last 4 days and inguinal swelling for last one year with dilated cardiomyopathy , heart failure.
On extreme high risk and iccu setting , hernioplasty was done.
The small intestine was found at the verge of gangrene. Procedure went uneventful under regional nerve block.
