যখন কোন ক্যান্সার কোষ, ব্রেস্ট-এ বাসা বাঁধে তাকে ব্রেস্ট ক্যান্সার বলে। ক্যান্সার শুধু ব্রেস্টে সীমাবদ্ধ থাকেনা দেরীতে ধরা পড়লে এটি পরবর্তীতে শরীরের অন্য জায়গায়ও ছড়িয়ে পড়ে এবং জীবনহানি হয়।

#ব্রেস্ট ক্যান্সার সম্পর্কে কেন জানা দরকার?

মেয়েদের শরীরে যত ক্যান্সার হয় তার মাঝে ব্রেস্ট ক্যান্সারের সংখ্যা সবচেয়ে বেশি।প্রতি তিন জন মহিলা ক্যান্সার রোগীর মাঝে ব্রেস্ট ক্যান্সার একজন।এ বছরের সমীক্ষা মতে আমরিকায় প্রায় ৩১৫০০০ জন মহিলা ব্রেস্ট ক্যান্সারে আক্রান্ত হবে এবং ৫০০০০ মত মহিলা এ ক্যান্সারে মারা যাবে বলে ধারনা করা হচ্ছে। কিন্তু আগেভাগেই সনাক্ত করা সম্ভব হলে হয়তো তাদের জীবনটা হয়তো অন্য রকম হতো।

সাধারণত ব্রেস্ট গঠিত হয় দুই ধরনের জিনিস নিয়ে - ডাক্ট ও লোব । এ দু'জায়গার যে কোন একটাতে ক্যান্সার হতে পারে।

#কেন হয়?

সঠিক কারন যদিও পরিষ্কার নয়।তবে বিভিন্ন ঝুঁকি আছে।এর মাঝে হরমোন জনিত, জীবন যাত্রা , পরিবেশগত কারন অন্যতম। অন্য কারন গুলো হল- বাড়তি বয়স, পরিবারের অন্য সদস্যদের কারো হবার ইতিহাস,অল্প বয়সে মাসিক শুরু ,দেরিতে মাসিক উঠা/মেনোপজ, প্রথম বাচ্চা ৩০ এর পর হওয়া বা নিঃসন্তান মহিলা,পিল খাওয়া, জেনেটিক কারন, অন্য কোন কারনে বুকে রেডিওথেরাপি পেলে, স্থুলতা।

#প্রতিরোধঃ

কিছু নিয়ম কানুন মানলে এর ঝুঁকি কমানো যায়।

- কর্মক্ষম থাকা ও ওজন নিয়ন্ত্রণে রাখা।

- কম চর্বিযুক্ত ও স্বাস্থ্যকর খাবার খাওয়া।

- সপ্তাহে কমপক্ষে ১৫০ মিঃ ব্যায়াম করা।

- ধূমপান ও উত্তেজক পানীয় পরিহার করা।

- সন্তানকে বুকের দুধ খাওয়ানো।

#প্রাথমিক পর্যায়ে রোগ সনাক্তকরনের উপায়ঃ

স্ক্রীনিংঃ

কিছু কিছু ক্ষেত্রে রোগের উপসর্গ হবার পর রোগ ধরা পড়ে।কিন্তু অনেকের উপসর্গের আগেই রোগ বাসা বাঁধে। । কোন ঝুঁকি আছে কিনা সেটা বিবেচনায় রেখে স্ক্রীনিং এর ধরন ঠিক করা হয়...

#১.সে্ল্ফ ব্রেস্ট পরীক্ষাঃ (সবার জন্য)

মহিলাদের এ বিষয়ে সচেতন করতে হবে যেন তারা নিজের শরীর সম্পর্কে জানে,শরীরকে চেনে এবং নিয়মিত ব্রেস্ট স্ক্রীনিং করায়।তার নিজেকে জেনে রাখতে হবে ব্রেস্ট স্বাভাবিক ভাবে দেখতে এবং হাত দিয়ে অনুভব করতে কি রকম এবং কোন পরিবর্তন লক্ষ করার সাথে সাথে যেন ডাক্তারের সাথে যোগাযোগ করেন। এই সেল্ফ ব্রেস্ট পরীক্ষা প্রতি মাসে একবার হলেও করা উচিত মেয়েদের। গোসল করার সময় বা অন্য সময়ে আয়নার সামনে দাঁড়িয়ে কিংবা শোবার সময় বিছানায় শুয়ে.. এই পরীক্ষা,নিজে নিজেই করা যায়। এ রোগ থেকে বাঁচতে হলে এটাকে জীবনযাত্রার একটা অংশে পরিনত করতে হবে এতে করে ৪০% ক্যান্সার প্রাথমিক পর্যায়ে ধরা পড়ে।

#২.স্বাস্থ্যকর্মীর মাধ্যমে ব্রেস্ট পরীক্ষাঃ

ট্রেনিং প্রাপ্ত স্বাস্থ্যকর্মী বা যে কোন চিকিৎসক এটি করে থাকেন। অনেক সময় রোগী নিজে পরীক্ষা করে বুঝতে পারছে না.. কিন্তু অভিজ্ঞ স্বাস্থ্যকর্মী, চিকিৎসকের কাছে আসলে অস্বাভাবিকতা থাকলে সেটি সনাক্ত হয়ে যায়।

#৩.ম্যামোগ্রাফিঃ

যাদের পারিবারিক ইতিহাস নাই, জীনগত ঝুঁকি নাই,কখনো রেডিওথেরাপি দেয়া হয়নি তাদের বলা হয় ব্রেস্ট ক্যান্সারের জন্য সাধারন ঝুঁকিপূর্ণ মহিলা।আমেরিকান ক্যান্সার সোসাইটির মতে প্রাথমিক পর্যায়ে রোগ শনাক্ত করার জন্য সাধারণ ঝুঁকির মহিলাদের নিম্ন উপায়ে ম্যামোগ্রাফির মাধ্যমে প্রাথমিক পর্যায়ে ক্যান্সার সনাক্ত করা যায়-

-৪০ বছর বয়সে ম্যামোগ্রাম নামক পরীক্ষা শুরু করা।

-৪৫ থেকে ৫৪ বছর বয়সের মধ্যে প্রতি বছর একবার করে ম্যামোগ্রাম করা।

-৫৫ এর পর দুবছর পর পর ম্যামোগ্রাম করা।

(ম্যামোগ্রাম হল ব্রেস্ট এর এক্স-রে।এটার মাধ্যমে ব্রেস্ট এর যে কোন চাকা যেটা চোখে দেখা যায়না বা হাতে ধরা পড়েনা তা নির্নয় করা সম্ভব।)

#ক্যান্সার উপসর্গঃ

অনেক সময় কোন উপসর্গ ছাড়াই ব্রেস্ট ক্যান্সার হয়।কিন্ত নিয়মিত নিজেকে পরীক্ষা করার সময় যদি নিচের কোন সমস্যা দেখা যায় তাহলে সাথে সাথে ডাক্তার এর সাথে যোগাযোগ করতে হবে।হতে পারে এর যে কোন একটা ক্যান্সার উপসর্গ।

-নিপলে ব্যথা

- ব্রেস্টে চাকা।

- হঠাৎ ব্রেস্ট এর চামড়ার পরিবর্তন।

-ব্রেস্ট এর চামড়ার লোমকূপ বড় হওয়া।

- হঠাৎ ব্রেস্ট এর আকার-আকৃতির পরিবর্তন হওয়া।

- হঠাৎ ব্রেস্ট ছোট হয়ে যাওয়া।

- দুটো ব্রেস্ট দুরকম আকারের হয়ে ওঠা ।

- নিপল হঠাৎ করে কোন দিকে বেঁকে যাওয়া।

- নিপল দিয়ে দুধ ছাড়া অন্য কোন রকম পানি বা রক্ত পড়লে।

- ব্রেস্টে ব্যাথা।

#কোন ডাক্তারের সাথে যোগাযোগ করবেন?

--নিকটস্থ হাসপাতালের মেডিক্যাল অফিসার বা সুযোগ থাকলে গাইনি বিশেষজ্ঞ বা সার্জারি বিশেষজ্ঞের কাছে প্রথমে যেতে হবে। যদি প্রাথমিক পরীক্ষায় ক্যান্সার বলে সন্দেহ হয় তাহলে যেতে হবে সমন্বিত চিকিৎসা কেন্দ্রে - মেডিক্যাল কলেজ হাসপাতাল বা সদর হাসপাতাল বা সার্জারি বিশেষজ্ঞের কাছে।এরপর ক্যান্সার বিশেষজ্ঞ ও সার্জারী বিশেষজ্ঞ এ দু'জনের সাথে পরামর্শ নিয়ে এর চিকিৎসা করতে হবে।

#রোগ নিশ্চিত করা যায় কিভাবে?

---যদি কোন চাকা ধরা পড়ে তাহলে এফএনএসি নামক পরীক্ষার মাধ্যমে সুঁই দিয়ে চাকা থেকে কোষ নিয়ে বায়োপসির জন্য পাঠানো হয়।এরপর রিপোর্টের মাধ্যমে সনাক্ত করা হয় চাকাটা ক্যান্সার কি না।

#চিকিৎসাঃ

কোন স্টেজে রোগ ধরা পড়েছে ও কোন ধরনের ক্যান্সার - তার উপর চিকিৎসার ধরন নির্ভর করে।চিকিৎসা পদ্ধতিগুলো হল---

**অপারেশন- এর মাধ্যমে শুধু টিউমার বা পুরো ব্রেস্ট-ই ফেলে দিতে হতে পারে।

**রেডিওথেরাপির মাধ্যমে ক্যান্সার কোষগুলোকে ধ্বংস করে দেয়া হয়।এটা অপারেশন নয়।

#অন্যান্য চিকিৎসা -কারো কারো ক্ষেত্রে নিচের চিকিৎসা পদ্ধতি দেয়া হয়-

--কেমোথেরাপি, হরমোন থেরাপি, টার্গেটেড ইমিউনোথেরাপি।

ব্রেস্ট ক্যান্সার সনাক্ত হবার সাথে সাথেই চিকিৎসা শুরু করে দেয়া উচিত। তবে জরুরি প্রয়োজনে দু'এক সপ্তাহ অপেক্ষা করতে ক্ষতি নেই। তবে ধরা পড়ার সাথে জেনে নিতে হবে রোগ কোন পর্যায়ে আছে এবং সে অনু্যায়ী কি কি চিকিৎসা নিতে হবে।

#অপারেশনের পর সন্তান ধারন সম্ভব কিনাঃ

অল্প বয়সী মহিলা যাদের ক্যান্সারের চিকিৎসা হয়েছে তারা অনেকে ভবিষ্যতে সন্তান ধারনের সম্ভাবনা নিয়ে চিন্তিত থাকেন।জেনে রাখা উচিৎ অল্প বয়সী ক্যান্সার রোগীর-

কেমোথেরাপি ও রেডিওথেরাপির কারনে ডিম্বাশয়ের ডিম্বাণু সংখ্যা কমে যেতে পারে। যার কারনে পরবর্তীতে বন্ধ্যাত্ব হতে পারে।তাই যারা পরবর্তীতে সন্তান নিতে চায়, সুযোগ থাকলে তাদের ডিম্বাণু নির্দিষ্ট ডিম্বানু ব্যাংকে রেখে দেয়া যেতে পারে।

নানা পরীক্ষা ও স্ক্রীনিং এর মাধ্যমে ৬৫% ক্যান্সার একদম প্রাথমিক পর্যায়েই নির্নয় করা সম্ভব। এবং সঠিক চিকিৎসায় ৯৯% কে সম্পুর্ন সারিয়ে তোলা যায়। এটা ভুলে গেলে চলবে না পুরুষেরও এ ক্যান্সার হওয়ার সম্ভাবনা থাকে।তাই নারী-পুরুষ সবাইকেই সচেতন হতে হবে স্ক্রীনিং-এর ব্যাপারে...তবেই ব্রেস্ট ক্যান্সার থেকে মুক্তি লাভ সম্ভব।
