Middle aged male patient presented with swelling of Lt Submandibular region acute pain aggravated by food intake fever etc...
High resolution USG reveals swollen vascularised Lt Submandibular gland( highest producer of saliva) caused by distal obstructing stone in its main duct.
So no flow saliva produced in the Lt Submandibular gland to the mouth for initial digestion & it's sequel.

Rare presentation.... a middle aged multigravid female with H/O recurrent bleeding P/V for few years ---- USG finding reveals a giant endometrial polyp with thick stalk & broad base arising from Lt lateral wall of lower uterine endometrial wall ------clearly seen within dilated endometrial cavity.{ Per operatively proved}.

Today i faced one of the difficult cases in my life.
48 yr old active apparently normal male patient was sent to me for USG of Neck to detect cause of intermittent swelling of ONLY Rt side of neck face upto Rt side of scalp especially after a long sleep..
Clinically & by high resolution USG study i could not find any abnormality except mild dilatation of Rt Internal Jugular Vein & mildly enlarged perivascular lymph nodes on both side of neck.
Taking clue from patient history & mild dilatation of IJV i scanned meticulously root of Rt side of neck as far deep as possible into Thorax......To my utter surprise i saw one medium sized elongated solid SOL peeping behind lower part of Rt Common Carotid Artery & Internal Jugular Vein pushing artery anteriorly.
Rt Internal Jugular vein also pushed to the Rt lateral side & dilated compared to the Lt side........indicating obstruction to venous return from Rt side of neck & head to the Rt heart especially during sleep......Everything is crystal clear & explained.

Rare presentation ...Primigravid young female ...detected incidentally on routine antenal USG check up having Bicornuate uterus showing common cervix common uterine body & two distinct lateral horns instead of common fundus.
Lt horn harbours regular gestational sac with a viable embryo & a yolk sac.
Rt horn is empty showing compact endometrium.

Fimbrial Ectopic pregnancy Lt side----rare case where ectopic gestational sac implanted in Fimbrial part of fallopian tube.......presented as sausage shaped ectopic mass surrounded by a layer of blood collection . ( Approx 1000 ml of blood in this case).

Rarest & deadliest form of Ectopic Pregnancy ....Interstitial (cornual) ectopic pregnancy detected close to Lt lateral side of fundic uterus well away from Lt ovary....it is still viable & unruptured. Situation like unruptured volcano.

PERIL OF HOMEOPATHY TREATMENT ESPECIALLY OF BREAST LUMP. Never do this mistake.
36 yr married nulliparous woman is on homeopathy treatment for her Rt Breast lump.
Long term homeopathy treatment usually musks most of the vital sonomammography findings to diagnose malignant etiology.
1st time USG guided FNAC done elsewhere failed to diagnose.
2nd time Guided FNAC done meticulously in my center & it comes out to be Rt breast duct carcinoma along with metastatic deposit in Rt axillary lumph node .Too late.
Vide scan picture .. mimicking giant fibroadenoma.
Dont be oversatisfied to see the patient age.
