Modest HbA1c Reductions Have Massive Clinical Impact

With Anti Diabetic Medications’ 1–2% HbA1c reduction looks small, but it’s clinically massive

• A 1% HbA1c reduction = about 30 mg/dL drop in average glucose.
• Each 1% drop reduces:
• Microvascular complications (retinopathy, nephropathy, neuropathy) by ~35–40%.
• Myocardial infarction risk by ~14%.
• All-cause mortality by ~20%.

That’s huge.
So what looks like “just 1%” actually means preventing blindness, dialysis, amputations, and heart attacks.

Fasting can drop glucose fast but not HbA1c
• HbA1c reflects average blood glucose over ~3 months (it measures how much glucose has attached to red blood cells).
• Even if you fast 36 hours and your glucose drops dramatically, your red blood cells still carry 3 months of glucose “history.”
• So, short fasting doesn’t change HbA1c much it changes real-time glucose, not the average glycation.
