Check your immunity against covid 🦠

60 year old male, Right parotid region swelling approximately 6x5x1.5 cm, superficial, well defined, soft, non mobile, non tender, multilobulated. Complain since a year with slow increase in size.No scan available. Case of Acinic cell Carcinoma reported- Value of FNAC..Pap and giemsa stain. (10X, 40X)

In a covid pandemic, any fever is Covid until n unless ruled out.

Widal test - titre 1:80 is not typhoid

Fever, cough, weakness, loss of taste or smell is covid, not typhoid or routine fever or general fever or seasonal fever.

Rising fever for >5 days or rising crp on 5th day, onset of breathing difficulty or spo2 less than 94 or 3 percent drop than previous spo2 is alarming.

CRP test- marker of inflammation or tells severity of infection.

Normal crp does not rule out covid.

Day 3 of illness - ideal time to get covid test.

Steroids on day 1 of illness - Never.

EARLY INVESTIGATIONS
1. CBC with NLR ( Neutrophil/Lymphocyte Ratio)

2. CRP,ESR,PROCALCITONIN
3. IL-6
4. D-DIMER ( Very important- Repeat after 2- 3 days)
5. FERRITIN
6. LDH ( A sign of cell death)


Notes-
1. CRP is a good marker to start steroids.
2. IL-6 is very unreliable.(Timely collection and rapid transportation required. Choose the same LAB)
3. D-Dimer------Monitor every 2-3 days
4. LDH – Useful follow-up parameter
5. NLR a good prognostic factor.
