"Please correlate clinically" — this line is often seen in radiological reports such as ultrasonography, CT, or MRI scans. It’s a vital reminder that diagnostics must be interpreted in the context of clinical findings, not in isolation.
As a homeopath, when I present cases of so-called incurable or surgically indicated diseases, skeptics often demand diagnostic and prognostic reports to support the claims. I welcome this, but in practice, there's a significant hurdle:
Most patients, once they feel better, do not return for repeat scans, citing several reasons:
Laziness or carelessness
Financial constraints
Fear of painful or uncomfortable procedures (e.g., CT cisternography)
Claustrophobic experiences during prior imaging
Belief that if a scan wasn’t needed for diagnosis or treatment, why repeat it?
The Case
A female patient from Shahjahanpur presented at my Lucknow clinic, complaining of long-term CSF rhinorrhea. She had already been evaluated by an ENT specialist who clinically diagnosed the leak. However, when a MR cisternography was performed, it failed to show any defect at the cribriform plate or skull base.
Yet, here was active CSF leakage—visible and recorded in the clinic—while imaging was normal.
This reinforces the importance of the phrase: “Correlate clinically.”
Imaging isn't infallible, especially in dynamic or intermittent leaks.
