FATTY LIVER - VERY COMMON about 10 million cases per year in India. In approx 1/4 th of patients if not restricted it can further progress to Cirrhosis and Liver failure.
FATTY LIVER CANNOT BE CURED BUT PROGRESSION CAN BE RESTRICTED . How ??
Main risk factor for fatty liver are Genetics, Obesity , Type 2 Diabetes and Alcohol intake . It may occur in people who take very little or no alcohol then it is called NAFLD ( Non alcoholic Fatty Liver Disease)or Metabolic dysfunction associated liver disease ( MASLD) .
In early phase no symptoms or complaint so goes unnoticed or detected during USG abdomen. In few people , NAFLD changes to NASH ( steatohepatitis ) here inflammatory cells do liver cell injury causing Liver dysfunction. Patient may feel upper abdominal pain , bloating, loss of appetite, fatigue etc . This is diagnosed with lab tests ( Liver function test LFT , Liver biopsy ) and other medical tests. In approx 1/4 th of patients NASH can further progress to Cirrhosis and Liver failure.

This can be restricted by lifestyle changes :
1. Reduce weight if obese
2. Add physical activity
3. Restrict alcohol intake
4. Control Diabetes and maintain normal blood Lipids ( cholesterol/ Triglycerides) levels

SCREENING TEST FOR DOWN’S SYNDROME and other genetic diseases of the baby when it is still in mother’s womb ( Prenatal screening tests ) are very important and essential to detect Genetic diseases in prenatal phase. It should be offered to All pregnant females specially the ones at high risk. These tests are done from mothers blood , Dual marker from 9-13.4 weeks of pregnancy and Quadruple test from 14-22 weeks of pregnancy.

NIPT is a newer highly sensitive and specific test in which foetal DNA from mother’s blood is extracted and analysed for Genetic diseases.
The test results are interpreted by experts and correlated with USG findings and clinical findings and further advisory is given to patients.

Adult Granulosa cell tumor o***y diagnosed incidentally postoperatively in radiologically normal o***y
67/F with postmenopausal bleeding ; USG Thickened endometrium with normal ovaries? Endometrial carcinoma. Underwent Hysterectomy with both tubes&ovaries;Histologically Endometrium show Hyperplasia without atypia but one of the grossly normal sized o***y show tumor with diverse patterns like diffuse solid microfollicular trabecular with typical Call- Exner bodies and nuclear grooves. Diagnosed as Adult Granulosa cell tumor o***y (pT1aNxMx)

GCT is a rare low grade ovarian s*x cord stromal tumor with relatively good prognosis and 20-30% chance of local recurrence after5-20 years of diagnosis. Though usually occur in post and perimenopausal female but can occur early in age too. Grossly diverse presentation may be solid , solid – cystic , only cystic ,white to yellow colour usually large size but may be small . The tumor is associated with hyperestrogenic symptoms so must be kept in mind in peri and postmenopausal patients with menorrhagia and thickened endometrium with different morphology of ovaries . Serum tumor marker Inhibin and Estrogen may be elevated .

31/F presented with missed periods for the last 4 months , not responding to Progesterone and Estrogen hormonal treatment. Endometrial curettage HPE shows collections of epithelioid histiocytes , lymphocytes , Langhan type of multinucleate giant cells admixed with necrosis and hemorrhage with destruction of endometrial glands - suggested of Tubercular Endometritis .
Ge***al TB is one of the common causes of secondary amenorrhea and Infertility in countries like India with high prevalence of TB. Ge***al TB usually spread through hematogenous route from other infective foci in the body. Early diagnosis and treatment with Anti tubercular drugs is mandatory to avoid long term complications.
