Tackling Coronavirus post lockdown: 3 simple steps
Lockdown doesn’t eradicate the infection, it just contains it & flattens the curve. In epidemics it is the herd immunity which plays a significant role in controlling the infection. And since most of our population doesn’t have immunity against this novel virus what will happen when people are allowed to move freely.
To deal with this threat 3 simple steps can be extremely useful:
1. Ease lockdown according to age and not profession.
Had coronavirus been a simple flu we wouldn’t have been worried. The reason we are worried about this virus is that it can be deadly. But if we look closely at the mortality figures there is a clear cut trend. Up to the age of 40 years the risk is 0.2%, 40-50 years 0.4% (and if you consider asymptomatic people who never had symptoms & were never tested the percentage would become even lower). In the age group 50-60 years 1.3% 60-70 years 3.6% 70-80 years 8% and above 80 15%. Also 80% deaths occur in people with preexisting diseases like diabetes, heart disease, asthma etc. Therefore for healthy people less than 50 years age (even better if less than 40 years) the risk of mortality is very low.
So if we allow only healthy people up to age of 40 or maybe 50 years to move about most of them will get only mild infection but the immunity they develop will protect the entire society. Remember we are fighting a war against a virus. And in war you allow the fittest soldiers to face the enemy.
2. Prevent Hospitals and healthcare facilities to emerge as hotspots for infection.
The reports of infection and deaths in healthcare facilities have already started pouring in. The risk becomes even graver when we consider that most vulnerable population (sick & elderly) will be exposed at these places. So separate hospitals need to be set up dedicated for the care of only coronavirus patients. These can be makeshift hospitals set up in buildings of educational institutes, marriage palaces etc. Having a dedicated coronavirus facility will prevent exposure to high risk group comprising other patients visiting routine hospitals. Also it will be easier to train the staff and provide protective equipment at these hospitals to safeguard the health workers. Even for quarantine such separate facilities can be created so that family members of suspect cases remain protected. The recovered COVID patients can be volunteers to look after the suspect/ confirm cases.
3. Blanket ban on Air conditioners
For any infection to spread there are 3 key factors- The Pathogen (coronavirus), the host (we human beings) and the environmental factors. While much can’t be done about the virus or human beings we need to study and better understand the environmental factors. If we look at the spread of coronavirus even though it has managed to spread across various continents and countries, within each country the spread is not uniform. It has spread mostly in urban areas while the rural areas have seen very few cases. It is possible that social distancing is more easily maintained in rural areas than crowded cities. However the most crowded areas in a city are the slum areas. Apart from Dharavi in Mumbai (where cases have been linked to tablighi jamaat congregation) infection has not broken out in other slum area. So apart from social distancing there appears to be other factors in urban lifestyle that help the spread of virus. One very important factor could be the air conditioning or central heating systems. The central temperature controlling units recirculate the air which can then spread to various parts of the building thus making it a fertile ground for virus to replicate. Thus if an infected person enters the building, he/she can infect all people in the building without even coming in contact.
As per an online survey of COVID patients done by our team most respondents had been in air-conditioned environment. Recently there was a report from Guangzhou China where infection in some patients was traced to air conditioners in a restaurant. With summers starting air conditioners are going to be used more & more. It is interesting to note that infection spread earlier in Kerala & Maharashtra where air conditioners are used earlier than in northern states. So a blanket ban on use of air conditioners and central heating units should be imposed immediately. Apart from virus getting recirculated it is also possible staying in open fresh air rather than enclosed spaces might have some antiviral properties but so far there is no scientific data to support this. Still it might be better to avoid the use of personal window air conditioners also.
