🧠 Rescue Angioplasty in Basilar Stroke – When Less Is More! : A 56-year-old male with posterior circulation stroke, basilar artery occlusion underwent mechanical thrombectomy with an underlying right V4 ICAD lesion causing critical stenosis.Rescue balloon angioplasty with tirofiban infusion. Vessel and flow remained stable. Patient extubated and neurologically intact except for left hemiparesis. A middle-aged female with an ophthalmic segment internal carotid artery brain aneurysm was successfully treated with flow diverter placement, achieving complete flow stasis. A 60-year-old gentleman with recurrent bihemispheric hemodynamic minor strokes (mRS 1) was found to have severe right cavernous ICA stenosis with complete left ICA occlusion. He was successfully treated with intracranial angioplasty and stenting of the right cavernous ICA, resulting in good opening and flow dynamics , therefore augmenting bilateral hemispheric perfusion. βœ… No recurrent ischemic events on follow-up

🫁 Catheter-Directed Thrombolysis for Pulmonary Embolism in a High-Risk Patient
A middle-aged female with recent MCA stroke on dual antiplatelets developed acute desaturation, tachycardia, and RV dysfunction. CT pulmonary angiogram revealed thrombi in both pulmonary arteries.
Systemic thrombolysis was contraindicated, so we performed catheter-directed thrombolysis.
βœ… Next day: Oxygen requirement improved significantly, with near-complete thrombus clearance on imaging.
This case highlights CDT as a safe, effective option for high-risk PE when systemic thrombolysis is unsafe.

🧠 Antiplatelet-Resistant Large Vessel stroke / Occlusion - MT awareness post
An elderly gentleman was rushed in with an acute basilar artery stroke. Despite being on antiplatelets, he had a large vessel occlusion, was unconscious, and on a ventilator.
With the effort of our stroke team, we performed an emergency mechanical thrombectomy.
βœ… First-pass TICI 3 recanalization was achieved.βœ… Within a week, he was off the ventilator and began to recover neurologically
