Spotted two rare embryologic vascular variants on cerebral angiography:

A. Infundibular origin of the mandibulovidian artery from the petrous ICA
B. Infundibulum of a persistent primitive dorsal ophthalmic artery arising from the cavernous ICA

These variants represent persistent embryonic carotid branches and are important to recognize to avoid misinterpretation as aneurysms and to prevent inadvertent embolization into orbital or skull base territories.

A 53-year-old male presented with recurrent posterior circulation strokes (NIHSS 3) with hemianopia and dysarthria due to vertebrobasilar insufficiency. He underwent successful basilar artery stenting with excellent recovery and no recurrence on follow-up.
⚠️ Early recognition + timely endovascular intervention = prevention of major disabling stroke

Transradial balloon-assisted thrombin injection → pseudoaneurysm occluded, flow restored, thrill returned, dialysis resumed.

A middle-aged CKD patient on hemodialysis presented with sudden loss of thrill in his only functioning AV fistula, with multiple prior access failures.
Imaging : Large iatrogenic brachial artery pseudoaneurysm causing flow steal and compression of the native artery → reduced distal flow & loss of thrill
Successfully treated with transradial balloon-assisted neck control with percutaneous floseal (thrombin) injection &.occlusion of pseudoaneurysm. Restoration of flow and thrill—dialysis resumed successfully

A 65-year-old patient presented with poor-grade subarachnoid hemorrhage (WFNS 5) due to a ruptured basilar apex aneurysm, complicated by hydrocephalus requiring EVD.

The aneurysm was treated using the WEB (Woven EndoBridge) intrasaccular embolization device, achieving immediate occlusion with preservation of bifurcation branches.

Privileged and honored to have been proctored by Uday Limaye during this procedure.

Honoured to have the opportunity to perform the first WEB treatment for a ruptured posterior circulation aneurysm in South Tamil Nadu.

Follow-up MRI showed preserved branch flow with no ischemia, and the patient demonstrated gradual neurological improvement with short-term antiplatelet therapy.

A collective team effort leading to a positive outcome.

🧠 Balloon-Assisted Coiling of a Ruptured PCoM Aneurysm
A middle-aged male presented with severe subarachnoid hemorrhage WFNS 5 due to a ruptured PCOM aneurysm. The aneurysm was treated successfully with balloon-assisted coiling.
Despite a difficult postoperative course, he improved neurologically with a functional outcome of mRS 3, currently requiring assistance mainly for tracheostomy care.
Brain aneurysm rupture is a life-threatening emergency. Early recognition and timely endovascular treatment can save lives.
