World Hemophilia Day 2022 is celebrated on 17th April all over the world to create awareness about this inherited bleeding disorder. The people with hemophilia have prolonged bleeding from any injury and their blood does not clot properly.

ELEVATION
Raise the arm or leg with bleed to a level higher than the heart (for
example, by using cushions or pillows) to help decrease the bleeding.

COMPRESSION
Put pressure on the area with bleed to “pinch off” the blood vessels and slow down bleeding.
This helps limit swelling to the injured area and keeps it from progressing to other parts of the limb.
A joint or muscle bleed is compressed by using an elastic (tensor)
bandage or an elastic compressive sleeve .

IMMOBILIZATION
The I in PRICE mean Immobilization. This involves using a
temporary half-cast or splint to limit movement. It helps rest the
part with bleed and prevents the child using an arm with bleed or putting weight on a leg with bleed.
• Immobilization should not be used for a long period of time
because joints become stiff and muscles become weak if they are not used. The injured joint or muscle must be examined by the HTC physiotherapist every 3 to 5 days to see when the cast or splint can be removed.
• The physiotherapist will also recommend exercises to help
restore joint mobility, muscle strength and balance.

ICE
Apply ice to an area with bleed to reduce pain and muscle spasms. Ice also helps reduce swelling and redness, also referred to as
inflammation.
You can use an ice pack, crushed ice in a plastic bag
or a bag of frozen vegetables wrapped in a damp towel.

Important points to remember about using ice include:
• Do not use ice over open cuts or scrapes.
• Do not use ice if there is poor circulation or poor sensation in the
injured area.
• Be sure your child can tolerate the ice. Some young children may
feel that the ice is more uncomfortable than the bleed.
• Do not leave the ice on longer than the recommended time.
Leaving ice on too long (more than 15 minutes) can cause muscle
weakness and can also cause an increase in blood flow

World Hemophilia day.
Every year, landmarks around the world are lit up in red as part of the World Hemophilia Day “Light It Up Red” campaign.
In Nagpur, Hemophilia society, Nagpur Chapter
with the help of sponsorship from
Kadu Healthcare Foundation
has taken up
“Light It Up Red” campaign.

In this campaign, the monuments in Nagpur city, like, Zero Mile monument, Gowari Smruti Smarak and Shaheed Smarak
Are lit up in Red colour to create and raising the level of awareness about Hemophilia.
The theme of big day this year is
“Adapting to change: sustaining care in a new world”.

REST
The affected area should initially be rested completely, generally for the first 24-48 hours depending on the severity of the bleeding episode. This allows the swelling to go down and prevents further bleeding.
The injured area should not be forced into any position, but instead rested in the most comfortable position possible.
Too much rest can also be damaging, so one must start to move the joint gently, as pain allows, within one or two days of treatment.

PROTECTION: Reduce weight bearing or stress on the affected joint or muscle by using crutches or other supports such as a 'collar and cuff' for the arm.
You may need to avoid putting weight on the affected side completely for the first 48 hours; and possibly longer if it is a severe bleed.

Clotting of blood involves a number of stages.
The blood of people with hemophilia
functions normally in the first three stages—
The constriction of the blood
vessels after injury.
The adhesion of platelets at the site of the injury and
the aggregation of other platelets and proteins to
plug the hole.
These three steps are usually sufficient to stop
bleeding from minor cuts.
It’s only the fourth stage––the formation of fibrin from the chain
reaction of clotting proteins like factor VIII and IX––which people with hemophilia lack.
This coagulation cascade is especially important to
stop internal bleeding.
