A middle aged male, admitted with severe abdominal pain, fever since 2 days. Blood reports and USG abdomen done, which suggested patient is suffering from choledochlithiasis ie biliary stone obstructing bile flow. We did ERCP to remove stone from bile duct. On ERCP stone was found to be in cystic duct compressing bile duct, Mirrizi syndrome. We put CBD plastic stent and free flow of bile re-established. Patient recovered well after ERCP from this acute phase and dischrged.

एक मध्यम आयु वर्ग के पुरुष को 2 दिनों से पेट में तेज दर्द और बुखार की शिकायत थी। रक्त परीक्षण और पेट की USG जांच से पता चला कि मरीज कोलेडोकलिथियासिस यानी पित्त के प्रवाह में बाधा डालने वाली पित्त की पथरी से पीड़ित है। हमने पित्त नली से पथरी निकालने के लिए ERCP किया। ERCP में पाया गया कि पथरी सिस्टिक डक्ट में थी जो पित्त नली को दबा रही थी, मिरिजी सिंड्रोम। हमने CBD प्लास्टिक स्टेंट लगाया और पित्त का मुक्त प्रवाह फिर से शुरू हुआ। ERCP के बाद मरीज इस तीव्र चरण से ठीक हो गया और उसे छुट्टी दे दी गई।

70 years old elderly lady, having metastatic colon cancer, presented with severe abdominal pain, found to have acute colonic obstruction. Patient was not operable for cancer as it was spread to distant organs. She did not want to undergo any surgery for her pain and obstruction. We opened up obstruction without surgery by putting metal stent across cancer growth and resolved the obstruction. Now patient was able to pass stool and her severe pain pain was resolved. Patient and her relayives were very happy and satisfied with results. Patient was dischrged and she went home very next day. By putting stent patient’s admission to hospital was cut short to one day only.

70 वर्षीय बुज़ुर्ग महिला को मेटास्टेटिक कोलन कैंसर था, उसे पेट में बहुत ज़्यादा दर्द था, उसे तीव्र कोलन अवरोध पाया गया। मरीज़ का कैंसर ऑपरेशन योग्य नहीं था क्योंकि यह दूर के अंगों तक फैल चुका था। वह अपने दर्द और अवरोध के लिए कोई सर्जरी नहीं करवाना चाहती थी। हमने कैंसर ग्रोथ पर धातु का स्टेंट लगाकर बिना सर्जरी के अवरोध को खोला और अवरोध को दूर किया। अब मरीज़ मल त्यागने में सक्षम थी और उसका गंभीर दर्द दूर हो गया। मरीज़ और उसके रिश्तेदार परिणामों से बहुत खुश और संतुष्ट थे। मरीज़ को छुट्टी दे दी गई और वह अगले ही दिन घर चली गई। स्टेंट लगाने से मरीज़ का अस्पताल में भर्ती होना सिर्फ़ एक दिन रह गया।

A 3 year kid swallowed a coin of Rs 5. Parents consulted family doctor who advised to wait for spontaneous passage in f***s. After 3 days parents brought him to me, on X ray abdomen coin was found to be in stomach only. We planned upper GI endoscopy and removed coin endoscopically.

Please don't give things to kids which are small and could be swallowed easily. As the coin was small, it passed ahead in stomach. Larger size could have stuck in throat or food pipe leading to ulcer and deep embedment. Batteries could cause severe ulcer and perforation. So it's always better to not let kids have small objects in their hands and near them.

Pancreatitis…
Usually presents with severe upper abdominal pain, classically radiates to back. Alcohol consumption, biliary stone are common causes. It needs to be treated with aggressive fluid management. Severe pancreatitis leads to necrosis and fluid collection. Large fluid pouches, infected or symptomatic ones need to be drained with endoscopic sonography.

65 yr/male had deep jaindice with cholangitis and sepsis. On ERCP patient had large ampullary growth. Due to large growth papila was not identifiable. Its very difficult to cannulate CBD. By god’s grace we managed to cannulate CBD, placed plastic stent for biliary drainage, bile was seen flowing freely. ✌️✌️🤟🤟

Cholangitis is l!fe thre@tening condition, in which establishment of bile drainage is very important. In such cases ERCP becomes an emergency procedure to relieve obstruction and maintain bile flow.
