This is a Cordeiro Type III B (Lateral segmental) mandibulectomy defect reconstructed with two segments of free fibula flap using our new modified technique to replenish soft tissue loss. We can achieve a completely normal soft tissue contour of cheek along with creation of buccal and lingual sulcus which are nicely maintained after adjuvant radiation.

Honoured to Participate as Faculty at APSICON 2025, Varanasi

It was a privilege to be part of APSICON 2025 — the 59th Annual Conference of the Association of Plastic Surgeons of India — held in the sacred city of Varanasi, the land of Kashi Vishwanath and Maharishi Sushruta, the father of modern Plastic Surgery.

I had the honour of presenting an operative video on Whole Breast Reconstruction in the prestigious CR Sunder Rajan Video Workshop. I also delivered a talk on our recent modification of the fibula flap for jaw reconstructions, a technique that has significantly reduced major post-radiation complications in our patients. I am grateful that both sessions were very well received by delegates and senior faculty alike.

A special highlight was the opportunity to get hands-on experience with the Da Vinci Robotic System for key reconstructive applications — an exciting step toward the future of Plastic Surgery.

The venue itself carried profound meaning. Standing in Varanasi, the land of Sage Sushruta, was a reminder of how deeply our specialty is rooted in ancient Indian wisdom and how far that inspiration continues to guide us today.

APSICON was also a wonderful chance to reconnect with mentors, teachers, friends, colleagues, and to meet new faces from across the country. I was glad to invite young and upcoming Plastic Surgeons to our centre BALCO Medical Centre for structured training in Reconstructive Microsurgery and Onco-reconstruction.

The conference kit included two beautiful souvenirs depicting Kashi and the legacy of Sushruta — treasures I will truly cherish throughout my career.

Grateful for the learning, the interactions, and the inspiration. Looking forward to carrying this momentum forward in our work for patients.

I recently had the privilege of being part of the Live Surgical Workshop in Head & Neck Cancers on Day 1 of the BMC Chhattisgarh Cancer Conclave 2025 at BALCO Medical Centre.

Two important surgeries were performed during the workshop:

1. A case of thyroid malignancy, demonstrated by none other than Dr. Anil D’Cruz, a pioneer in Head & Neck Surgery.

2. A case of tongue cancer, resected by Dr. Gauri Pantvaidya, Professor & Head of the Head & Neck Division, Tata Memorial Centre, Mumbai.

I had the honour of demonstrating tongue reconstruction in the second case, using a free ALT flap. During my session, I focused on:

1. Flap planning in near-total glossectomy to optimize functional outcomes

2. Working under the operating microscope

3. A step-by-step approach to flap transfer, inset, and microvascular anastomosis

4. Post-operative protocols for flap monitoring and patient care

The workshop was attended by numerous Oncosurgeons and Plastic Surgeons from across the region and other states. The discussions were greatly enriched by eminent moderators, making it a highly interactive and fruitful experience.

It was a special privilege for me to operate alongside stalwarts who have been an inspiration for the next generation of surgeons. Following the surgeries, I also participated in a panel discussion on Head & Neck Cancer, where I emphasized the vital role of Plastic Surgeons in comprehensive cancer care and reconstruction.

I am also glad to share that both patients from the live surgical workshop have recovered well from anesthesia and are stable. We remain hopeful for their speedy recovery and extend heartfelt gratitude to them for allowing us to treat them and contribute to surgical education for our peers.

A sincere thanks to the organizing committee for this opportunity. It was truly an honour to be a part of such a landmark academic event.

🌟 Conference Reflections – APMPCG Jabalpur 2025 🌟

I recently had the privilege of attending the Association of Plastic Surgeons of Madhya Pradesh and Chhattisgarh (APMPCG) Conference held in Jabalpur.

I had the opportunity to:
🔹 Deliver a talk on a simple modification of free fibula flap, which has helped us achieve excellent results in preventing facial soft tissue atrophy and avoiding bone/hardware complications post-radiotherapy.
🔹 Chair two sessions – one on Head & Neck Reconstructions and another on Venous Ulcers.
🔹 Participate in a live workshop on Botox and Fillers conducted by the renowned Aesthetic Surgeon, Dr. James D'silva, which offered valuable insights into achieving optimal aesthetic outcomes.

The two-day event was filled with outstanding presentations by stalwarts in our field, knowledge-sharing, and vibrant discussions. Beyond academics, the evenings were equally memorable – from delightful cocktail dinners to impromptu musical performances by talented colleagues that made the atmosphere truly special.

My heartfelt appreciation to Dr. Pawan Agarwal, Dr. Rajeev Kukrele, and the entire Jabalpur team for putting together such a compact, flawless, and thoroughly enjoyable conference. The hospitality was exceptional.

A special highlight for me was my interaction with Dr. Dushyant Jaiswal (TMH, Mumbai), whose work in advanced head and neck reconstructions is inspiring. His personal words of encouragement felt like those of an elder brother, motivating me further in this journey.

Lastly, Jabalpur has always been close to my heart, and this meeting only deepened that bond. Looking forward to the next opportunity to meet this wonderful fraternity again!

In Plastic and Reconstructive Surgery, we have the concept of TISSUE DISPLACEMENT and TISSUE REPLACEMENT.

When there's a tissue loss resulting in a defect, we can fill in the defect by TISSUE DISPLACEMENT if there's adequate expendable tissue in the adjacent area. The tissues surrounding the defect can be moved around to fill the defect without having any difficulty in repairing the donor site.

If the tissues adjacent to the defect are not adequate, we fill in the defect by TISSUE REPLACEMENT. Here, we harvest a tissue from a distant site and fill the defect.

This elderly lady had a defect involving the lower lip and left oral commissure. Her mouth opening was excellent with laxity of the oral mucosa as well as the facial skin.

I consider the whole face as a combination of tissues, divided into various sub-units. When there's oral submucous fibrosis (OSMF), the whole facial tissue (oral mucosa as well as the perioral skin) tend to become sunken and inadequate to allow tissue displacement for reconstructions of facial defects.

When the facial soft tissues are loose and lax, it provides a great indication for tissue displacement. The defect here was reconstructed by a simple nasolabial flap. The mouth opening is well maintained along with a loose and supple scar. This can still be considered a facial soft tissue displacement although we are replacing the lip sub-unit tissue with an adjacent skin sub-unit tissue.

In severe long standing OSMF, along with a lip defect, achieving this kind of result with a simple nasolabial flap is difficult and may need to be addressed by importing distant soft tissues.

On this special day, I bow in gratitude to all my gurus who moulded me into who I am today. A teacher is not just someone in a classroom—they can be our mentors, our colleagues, our juniors, and even our students—each shaping us in unique ways and helping us grow into better leaders in our respective fields.

As a doctor, some of my greatest teachers have been my patients. They teach resilience, courage, and the true meaning of healing every single day. Their lessons go beyond textbooks and continue to inspire me to strive for excellence and compassion in my work.
