Diabetes doesn’t start overnight. Long before your blood sugar begins to rise… another silent process has already been damaging your health. That hidden culprit is called Insulin Resistance (IR).

IR develops years before diabetes or even prediabetes show up on blood tests. A critical, yet often overlooked, link exists between chronic pain and su***de. WHO recognizes chronic pain as a key individual risk factor for su***de. Several clinical guidelines also highlight chronic pain as a potential risk factor for suicidal ideation & attempt. Patients with chronic pain are 2–3 times more likely to experience suicidal thoughts and attempts. Suicidal thinking ranges from fleeting wishes (“not wanting to wake up”) to detailed plans, all of which are red flags for urgent support.

It is crucial to differentiate chronic pain from acute, protective pain stemming from sources like injury, inflammation, surgery, cut, burn, etc. Chronic pain is a distinct entity where the nervous system's alarm remains active long after the initial cause has resolved. Chronic pain is a serious, long-standing, and debilitating condition, affecting 1 in 5 people worldwide. Beyond the physical suffering, it brings a heavier economic burden, poorer quality of life, worse mental well-being, and decreased physical functioning.

As a pelvic floor physiotherapist, I see this reality firsthand. The link is especially strong in chronic pelvic pain (CPP). Suicidal ideation is alarmingly more common in CPP. In one study, 23% of IC/BPS patients reported suicidal thoughts in just two weeks—compared to 6% of healthy controls. Anhedonia (inabilityto feel pleasure or motivated which were pleasurable earlier) seen in conditions like endometriosis. Overlapping pain syndromes—IBS, CFS, interstitial cystitis/painful bladder syndrome, endometriosis, fibromyalgia, vulvodynia, migraine, chronic low back pain, TMD—further increase vulnerability.

Since chronic pain is fundamentally different from the pain you get from a arthritis, post surgeries, etc., it requires a comprehensive approach from multidisciplinary collaboration working together on biopychosocial aspects. Physiotherapy is very important part of the team but not traditional pain & inflammatory relieving strategies work because often there is no local tissue damage.

Recent studies tested a new, non-drug treatment for men with lifelong premature ej*******on (PE). It used gentle electrical signals (called Transcutaneous Electrical Stimulation or TES) applied to the perineum (area between the ge***als and a**s) or at ankle.
