Sharing a rare and a challenging case of :

*Laparoscopic Right Adrenalectomy for 15 x 10 cm MyeloLipoma of a Right Adrenal Gland.*

34 yr male presented with Recurrent pain in the Right hypochondrium from one month.

On evaluation -
Radiologically there is a 15 x 12 cm MyeloLipoma in the Right Adrenal Gland.
Harmonal analysis showed it’s a non functional tumour.

A Laparoscopic Right Adrenalectomy was done .Intra operatively the tumor was densely adhered to Liver and the IVC .

Operating time : 3 hrs.
Blood loss : 50 ml.

Intra Op and Post Op period was uneventful and the patient was discharged on POD 3.

Sharing one interesting case of Angiodysplasia of colon with massive lower GI Bleed of approximately 3 litres-

40 yr male presented with severe lower GI Bleed from few hours approximately 2 litres.He is a known cardiac patient with history PTCA and on anti coagulants.

Haemoglobin was 4 gm and he was requiring high dose ionotropes.

Emergency colonoscopy was done which showed around 2 litres of blood in colon with active bleeding point and spurting from hepatic flexure of colon.

Emergency laparotomy was done with intra op ENTEROSCOPY to localise the lesion.There was a pinpoint angiodysplasia in the colon which was the source of the bleed.Wide excision of the lesion was done.

Patient improved post op and was discharged on POD VI.

Pancreatic surgeries are very dreadful .Times have changed from surgeons being afraid to touch the pancreas to being able to perform critical life saving surgeries on pancreas.
