The BLEED....
45 yr old male with CT showing exophytic SOL from Thyroid with intra-tracheal extension.
Bronchoscopy showed the lesion which started bleeding on touch only!!!
Managed with cold saline and IV Tranexa. Stable on table, recheck Bronch didn't show any active bleed and discharged same day with referral to higher center for surgical removal of the vascular tumor.

Beyond your vision... there is still some hope...

A 9 year old girl with history of some foreign body aspiration one month back.

Under TIVA (Total IV anesthesia), flexible video bronchoscopy was done via oral route which showed granulation tissue in the Right Middle Bronchus. FB forcep was tried but could not get anything blindly and hence we used C-arm to locate the FB body and retrieved it with toothed forcep under the C-arm guidance. It was Button cell battery.

(Had rigid scope for the back up if failed with the flexible scope)

Prefer Gulal which contains corn-starch blended with FDA approved dyes
Avoid colors with chemicals & heavy metals (as it causes respiratory complications)

If u have any COVID like symptoms or had so recently...
Then avoid vaccination for 3months or get ur COVID test done before vaccination.
As the vaccine will flare the infection.

Although it is decided by our DM and other administrative officers abt the criteria to diagnose a COVID patient and disposal of the same on the basis of the RT-PCR or antigen report, I have a case where patient showed positive report after he expired. His RT-PCR and antigen 10days back were negative. His CT thorax done at that time was suspected of COVID with CO-RAD 5 and CT CT severity score of 16/25.

As patient didn't get admission to AIIMS-P due to negative report he was admitted at a pvt hospital although he was in financial crisis.

Out medical faternity has no power to make a criteria rather a administrative one who even doesn't know the ABC of medicine!!!
This our great India and so we sud feel proud of being Indian
