Sharpey's Test for TFCC Pathology

Sharpey's Test is a provocative clinical assessment used to evaluate the integrity of the Triangular Fibrocartilage Complex (TFCC) and identify signs of ulnocarpal abutment syndrome. It serves as an important tool in diagnosing ulnar-sided wrist pain, particularly when symptoms may be related to TFCC injury or excessive ulnocarpal loading.

The TFCC is a key stabilizing structure located on the ulnar side of the wrist. Composed of fibrocartilaginous tissue, it functions as a shock absorber between the distal ulna and the proximal carpal row, while also contributing to DRUJ stability. Ulnocarpal abutment occurs when increased contact between the ulnar head and the carpal bones—often due to positive ulnar variance, degenerative changes, or TFCC lesions—leads to pain, inflammation, and mechanical restriction.

Sharpey’s Test helps clinicians differentiate between TFCC pathology and other causes of ulnar-sided wrist pain by reproducing the patient’s symptoms through controlled load and rotation.
