Understanding the angles:

1) Intramuscular (im) : injects deep into the muscle at 90Β°.
2) Subcutaneous (sc) : goes under the skin at 45Β°.
3) Intravenous (iv) : enters the vein at 25Β°.
4) Intradermal (id) : shallow under the epidermis at 10-15Β°

What Flying does to your body?

Most people think flying is just sitting in a chair in the sky ✈️ but is it ?

What happens during a flight:
β€’ Cabin pressure drops β†’ oxygen levels fall
β€’ Heart has to work harder to deliver the same oxygen
β€’ Cabin air is drier than the air on surface β†’ you lose water in every breath
β€’ Blood volume goes down because of dehydration β†’ blood gets a little thicker
β€’ Sitting still slows circulation β†’ legs get heavy + swollen increases the risk of venous thrombosis
β€’ Airway lining dries β†’ immune system barrier weakens leading to cold.
β€’ That β€œjet lag” feeling?
Is not just time zone difference, it is physiological

What can be done to protect myself (every flight):
β€’ Hydrate before boarding (start ahead, not behind)
β€’ Skip salt + alcohol in the air (both thicken blood)
β€’ Stand / stretch every hour to keep circulation moving
β€’ Use saline nasal drops before takeoff to protect nasal mucosa
β€’ Walk after landing to reset blood flow + oxygen delivery

Tiny habits create little difference in your mid air health.

Elderly patients and their challenges!

As the doctor in charge we should be bringing out the patient, in active mode, not in a bed ridden status.

"The best way of preventing a cascade of crisis is by keeping elderly away from un-necessary investigations, keeping them away from over therapies, drugs, health rejuvenator remedies and let them be within their own natural sphere of activity".

"Not all abnormal needs to be treated at the fag end of life, rather it may be a part of their life". An ambulatory oldie with an indwelling urinary catheter is far better than a chairbound elderly entrapped in a whirlpool of laboratory investigations and imaging.

Accept what 'age' gives us rather than trying to treat 'age'.

Morbidly obese elderly woman weighing ~150 kgs, Diabetic, Fat Embolism syndrome, Obstructive sleep apnea, breath holding time of 15 secs, Sat dropped to 30% during apneic episodes in sleep, 45 cm neck circumference with thick nape of neck padding, posted for fracture left proximal humerus head fixation and rt femur shaft fracture nailing. Case managed using Regional Anaesthesia techniques using PNS and CSE for post op care. Choice of drugs in multimodal analgesia, blood transfusions, diabetic control, ABG corrections and prophylaxis for DVT played an important role.
Difficult airway, fear of ventilator dependency, uncontrolled DM and DVT were the critical demands.
Patient got operated and is happy to be pain free and active πŸ™

Elderly patient posted for surgical intervention on a recurrent brain tumor, post radiation therapy. She had a restricted mouth opening of 1.5 finger and MPC 4. With the use of advanced VLS we could visualise the cords and intubate her to protect her airway during the efforts to remove the tumor.
