Which: Obviously Human milk. A baby is born with an instinct to suckle so Breast Feeding is the ideal food for it.
All formula feeds are cow milk based which is difficult to digest for human babies n can cause GERD, constipation, colic, allergy etc. However they are semidigested n fortified with vitamins n minerals.
Whole cow's milk is ideally to be given only after 1 yr. It is better than buffalo milk which has high fat content n more difficult to digest. Never add any water to milk, it reduces it's calorie content so baby doesn't put on weight.

How to give?
Breast suckling is ideal, milk at body temperature goes straight into baby's mouth without any risk of infection
Wati-spoon: Sterilised steel wati spoon is good to feed small amount of milk but baby gets frustrated in between n milk leaks out of its mouth while swallowing.
Bottle : Sucking at a bottle is easier n convenient. However bottle should be sterilised before every feed n left over milk to be thrown away. It has higher risk of stomach n ear infections.

How much: Breast feeding is Demand feeding i.e baby should be fed as n when it cries n as long as it wants. A full term newborn baby can take 50-60ml every 2-3 hrs. As the baby grows it can feed 90-120 ml every 3-4 hrs. In 24 hrs a baby feeds about 8-10 times. Never force feed a baby n burp well.

How long to feed: Milk is the main nutrition for a baby less than 1yr of age. Ideally baby should be exclusively breast fed till 6mon then weaning should be started with solid foods n not more milk. By 6 mon baby is taking 8-10 milk feeds, it should come down to 3-4 by 9 Mon n only 2-3 by 1 year of age. After 1 yr a baby shouldn't take more than 400-500 ml milk in all.
Solid food should be started gradually from 6 mon 1-2 feeds n gradually increased to 4 times by 1 yr.
A baby can sleep through out night without feeding by 6-7mon, so night feeds should be decreased n stopped by 9-10 Mon n definitely by 1 yr.
Continuing frequent breast or bottle feeds after 1 yr causes many problems
1. Anemia: Milk doesn't have iron so too much milk leads to low hemoglobin
2. Dental caries: Milk remains near teeth all the causing mainly the top four to decay
3. Difficult weaning: A baby likes to drink milk which is easy n fast compared to sitting n eating food which takes more time. So it avoids solid food n asks for milk all the time. After 1 yr a baby enters the Terrible Twos period n becomes very jiddi or adamant. Weaning breast or bottle feeding becomes very difficult.
So suckling at breast or bottle should be stopped by 1 yr n milk should be given by sipper or cup or small edged glass. We advise to just throw the bottle in front of the child n stop it. The child throws tantrums n cries a lot but don't give in. After few days of not drinking milk it will gradually come around. The solid food intake increases once milk is reduced.

Every baby is different n these r general guidelines by which to manage ur baby's graduation from milk to solid foods without losing weight n patience.

In my day-to-day practice mothers often complaint that her child is not eating and looks very thin. For some strange reason we all want chubby kids.
But chubby is not healthy. In our efforts to make kids chubby we force feed n overfeed them, even junk foods with the belief that atleast the child is eating.
This n easy availability of tasty junk food has led to the epidemic of OBESITY in kids.
An obese kids parents are also obese. But they continue to be in denial. Everyone thinks that there is some medical which has caused obesity, but this is so in only
1 in 100 kids whereas in 99% the cause is nutrition n exercise imbalance.
Going to endocrinologist, doing lots of tests, medicines will not help.
So what's the solution?
1. Acceptance- Accept the fact that ur child is overweight/ obese. Plot actual weight on the weight graph, calculate BMI. Figures don't lie. Nobody gets obese without reason.
2. Identify the reason: Eating healthy food in proper quantity is the key to maintaining average weight. Eating too much healthy food will also lead to obesity. Controlling portion size is equally important.
3. Diet- Kids have growing body so they shouldn't be made to fast. They should be given healthy food options for all their meals. Fruits, vegetables, sprouts, dry fruits should be given for snacks rather tban biscuits, Chips, pizzas, burgers, etc.
High calorie food items made of too much milk, maida, sugar, oil, ghee/ butter, cheese should be limited. Small frequent healthy meals help to maintain good weight.
4. Exercise- Every child should be physically active. Limit screen time-TV, mobile. They should walk, run, play, swim, dance, climb stairs n not sit n chit-chat. Enroll in some sports classes. Kids should be physically active atleast 1 hour daily.

As soon as child starts getting overweight parents should start taking these measures to avoid obesity n related complications in future. The entire family should adopt a healthy lifestyle rather than forcing child to change alone.

Fussy Eaters_kids

It is quite normal for kids to be fussy about food. Babies are born with reflex to suckle at breast and swallow milk. Everything else like drinking from cup, chewing and swallowing solids have to be learnt. So teaching a child to eat different types of food while he/she is growing n developing is no mean task.
Following are some guidelines which will help to incultate healthy eating habits n avoid fussiness.
1. Start early: Weaning should be started by 4-6mon. Small quantities of soft mashed solids should be given at regular times, no force should be used. Meal times should be fun. Talk or sing to baby while feeding. 6-12mon is a good time to introduce different types of semi-solid home cooked foods- cereals, pulses, veggies, fruits etc. Babies may not like new things in the first attempt but keep persisting. Eventually taste will develop n baby will start accepting new foods.
2. Timetable: Meal times should be regular n at fixed times. Baby should always be in sitting position n immobile. Using a high chair from 8-9mon onwards is a good idea. As soon as u place a baby in high chair it knows that it is going to be fed. Baby learns to eat while sitting in one place. Avoid running behind while feeding. If u want ur child to develop healthy eating habit a discipline of meal times has to be inculcated early on.
3. Distractions: Like TV, videos on phones are to be avoided. These r habit forming n child will not eat without it. Also such children dont know what they r eating, they just swallow. Too much noise n too many people around also distract the child. It is important that the child is calm n focused on eating. Eating along with family helps the child to learn to eat what his/her family eats.
4. Choices: Offering too many varities n choices confuses the child. Insist on eating the same food as rest of family. Giving too much milk or snacks keeps the child full who then refuses meals. Children imitate parents, so eating fruits, veggies in front of them encourages them to copy n eat the same.
5. Praises n Punishments: Praising on eating well is good. Hugs, kisses, stars r good enough, no need to bribe with sweets. As far as possible ignore bad behavior during meal times, dont smack or yell. Be firm n strict but calm. Too much pampering n independence is also not good, parents have to teach children to listen n follow them.

In some kids the skin becomes dry, itchy with red spots or patches. This could be eczema or atopic dermatitis wherein the skin barrier is leaky and it reacts severely to various triggers.

Common Areas affected: cheeks, around mouth, elbows, back of knees
Appearance: Rash is red spots few spread out or join to form patch. Extremely itchy n might ooze or bleed on scratching.

Triggers: Could be anything like cold, dry weather (winter), creams, lotions, soaps, detergent, hand wash liquid, oils, gram flour (babies), milk, eggs, nuts, fish, wheat, medicines etc. The list is big. A child could be allergic to anything. A well maintained diary about changes in diet or skin products could help in identifying the allergen. Avoiding triggers or allergens help to avoid flare-ups in eczema.

Treatment: The mainstay of treatment of eczema is "moisturise-moisturise-moisturise". Keeping skin soft n supple keeps rashes away. It is very important to apply water based lotion atleast once daily thru' out year as Pune's weather is dry. Apply immediately after bath on moist skin, it helps to retain water in skin. Those suffering from eczema should moisturise atleast 3-4times escpecially on dry patches with thick oil based lotions or creams like atogla, cetaphil, oilatum.
Use syndets like tedibar, oilatum, sebamed or neutral-pH soaps like dove, pears, etc to avoid dryness of skin.
If eczema still flares up, steroid based creams is required to reduce it. Oral medicines may also be required to control itching.

Hypothyroidism in kids
All of us have a butterfly shaped thyroid gland in centre of our necks. It secretes thyroxine hormone which is very important.
Sometimes this thyroxine is produced less or completely absent, then its called "Hypothyroidism"

Congenital Hypothyroidism: (Cretinism)

This is absence of thyroxine by birth. It can be because there is no thyroid gland at all or it malformed or not functioning well.
It has nothing to do with mother's hypothyroidism.
Nowadays baby's thyroid levels are checked after birth. This is to diagnose and treat congenital hyporthyroidism as early as possible to prevent permanent mental retardation.
If not treated early these Symptoms are seen:
Dull lethargic baby, not crying frequently, not feeding well, constipation, swelling in neck region, skin-dry, cold, thick, rough n pale, floppy, anemic, not gaining weight, not developing well, mental retardation.
If treated early with thyroxine tablets baby develops normally n no permanent damage occurs.
Blood levels have to be checked frequently n treatment n growth monitered. Stopping treatment abruptly without consulting doctor can have drastic consequences.

Acquired Hypothyroidism:
This happens in later childhood especially teenage girls. Thyroid gland stops functioning well because of various reasons n thyroxine levels drop.
Symptoms:
Lethargy, dullness, weight gain, hoarse voice, thick cold dry skin, feeling cold, slow pulse, mood swings, depression, anemia etc.
Simple blood test can diagnose the problem and Simply taking thyroxine tablets regularly everyday can solve it.
But checking blood regularly to monitor thyroid n continuing with medication is very important.
Hormonal problems like hypothyroidism, diabetes tend to be long lasting to permanent. They need regular treatment n stopping medications without doctor's advice can be dangerous.

Dengue

There is an increase in cases of Dengue in Pune recently. People are naturally worried n want to know how dangerous it is n how to prevent it.
Dengue is a 'viral infection' spread thru bite of only infected female Aedes mosquitoes. So all mosquito-bites will not result in dengue.
High fever(102-104F) is the most common symptom with headache, eye pain, joint n muscle pains, rash, vomiting, stomach pain in some.
Blood test is the only way to diagnose it.
There is no specific anti-viral medicine that can kill n cure dengue. Controlling symptoms with medicines n looking for complications is what doctors do. Majority of patients (first time dengue cases) recover without any complications. Some patients end with serious complications like bleeding n shock. To prevent this hydration i.e fluid intake should be good. Monitoring hematocrit (blood concentration) n platelet count is important. Normally platelet count is above 1.5Lakh. Even if it falls to very low levels like below 20000, it is okay if there is no bleeding.
Dengue can occur again n it has more chances of complications than the first time.
Home remedies like papaya leaf juice, kiwi, dragon fruit etc dont help. Platelet count increases naturally in few days when patient starts to recover.
There is no effective treatment for dengue neither a vaccine. So mosquito control is the only way to prevent it.
1.Avoid accumulation of water in n around ur house in plants, broken bottles, coconut shells, tyres etc
2. Spraying on insecticides in open areas in societies
3. Wearing full sleeves-pants clothing
4. Applying mosquito-repellant creams, roll-ons etc
5. Using nets on windows, to sleep-in

Recurrent Infections in Children.

Currently with the prolonged monsoon we are seeing increase in infections in children. Most infections are viral including influenza, Dengue.
Children especially less than 5yrs have weaker innate immunity hence are more susceptible to recurrent infections.
1. Starting school/ daycare early
2. Having an older school going sibling
3. Frequent changes in weather
4. Frequent travel
5. Poor nutritional status
6. Genetically weak immunity
7. Incomplete vaccination, especially Flu vaccine, pneumococcal vaccine
These are some of the reasons why some children fall sick frequently. Also they don't recover soon n require antibiotics.
We don't have any control over genes or weather, but we certainly can try to change other reasons.
1. Improve diet and weight of kids by giving healthy foods like milk, all fruits, all vegetables, dry fruits also eggs, fish, meat if possible.
2. Give all vaccines available, atleast those diseases can be prevented.
3. Avoid unnecessary travels, picnics in bad weather.
4. Avoid eating out n unhealthy foods
5. Avoid putting child in daycare or school early as far as possible.

Mobile Addiction in Children

Children are spending more n more time on the mobile phones lately. This has become a common problem nowadays n is affecting their development n mental health.
We as parents have also contributed immensely to this menace. As soon as a baby is born we rush to take its photos. Many more selfies n photos of milestones n activities follow. We r also guilty of showing them rhymes, cartoons, videos to either distract, coax, entertain, feed or keep them busy.
As kids grow they become adept at handling the smartphones, playing games, watching videos or making friends on the various social media sites. They start living more of virtual life than the real one with friends n family.
Being on the phone constantly affects the eyesight, hearing, communication n social skills, language development. Learning difficulties, anxiety, depression, aggression n other mental health issues can develop.
They are at risk of getting exposed to violence, adult content online or fall victim to s*xual predators.
The solution lies in prevention. We must first n foremost restrict our own use of mobile phones especially in front of our kids. We should talk, sing, play n spend time with them instead of giving them phones. Explain pitfalls of this addiction to older kids n encourage them to spend time with real friends. Get them interested in sports.
Use parental locks n controls on our phones.
No mobiles for toddlers n only 1/2 hour for older kids n buying their own phones after 15yrs should be the norm.
Children who r already addicted to mobiles or having any mental health issues should be taken to child psychiatrist for counselling n help.

Part 2: Problems during Puberty

Girls might face some common n minor problems with the onset of puberty like

1. Tenderness or pain in the ni**le or breast area. This is nothing to worry about. Pain gets better with time n needs only hot water bag or a pain killer for relief.
2. Pimples, white or black heads develop on face, arms because of increased hormones which cause increase in oiliness and sweating. Washing face frequently n not touching it frequently helps. Excess use of oil on head n oil-based creams should be avoided.
3. Hair in armpits n bikini area can be trimmed with trimmer or scissors. Use of wax or harsh chemicals should be avoided.
4. Increased sweating can cause fungal infections in creases. Bathing twice especially after sports helps.
5. Irregular me**es is common in the first few years. Either the cycles are irregular or the bleeding is scanty or heavy. Nothing much can be done here as they are hormone-driven. However this can cause anemia n weakness in girls n needs consultation with gynaecologist. Mild pain in back n lower abdomen can be managed by hot water bag, might need pain killers if pain is debilitating.
6. Weight issues: Generally height increases rapidly in the first few years n slows or stops after period starts. When that happens girls can start putting on weight. Hence it is important to maintain healthy-eating exercise routine thru'out.
7. Emotional Issues: As the body is changing so is the mind. Hormones play havoc in adolescents. Emotional outbursts, arguments, defiance become common. A little understanding, patience n open communication is all that is needed to help teenagers tide over this phase in their life.

Puberty in Girls

Normally around 8-9yrs girls start to show signs of puberty, 1st sign being tingling or pain in ni**le area, following which breast starts to grow. 2nd sign is appearance of hair in arm pits n bikini area. Growth of breast, hair n s*x organs, rapid increase in height go on for 2-3yrs n finally menarche or me**es start. So periods is usually the last to happen around 12-13yrs n they also take some time to become regular. Thereafter the height of girls doesn't increase much.

Earlier periods would start around 13-14yrs of age but nowadays with good nutritional and socioeconomic status girls are maturing faster n menarche age has come down to 10yrs. There is nothing wrong with this n doesn't need treatment. This usually happens with overweight girls n they remain shorter also as a result of early puberty. Hence adopting a healthy lifestyle i.e eating healthy foods, avoiding junks, playing outdoors, exercising regularly will help in good health n height.

Precocious Puberty
Only if the puberty starts too early i.e periods by 8yrs it is labelled as precocious. These girls need to be checked n treated by pediatric endocrinologist. It involves several blood tests n painful injections n might still not be able to delay puberty. Routine hormonal injections to delay puberty so that height can be increased is neither advised nor safe.

Every year in spring time there is a surge in viral infections like inflenza, chickenpox etc. Vaccinations are available and can easily prevent them and reduce their severity.
Chickenpox can strike at any age and is bad in older kids and adults. Children have to miss school n play for 2-3weeks and suffer fever, pain n itching. Once the lesions start there is no medicine to stop it and cure it. Sometimes the painful fluid filled rash covers the entire body. Lesions can become infected, pneumonia, encephalitis can also occur.
In this day and age when a vaccine is available to prevent an illness, use it. Vaccination can prevent both chickenpox and herpes zoster( caused by same virus later on).

Foreign body Ingestion

Small kids in age group of 1-5years often tend to put small objects like peanuts, crayons, beads, pins, coins, batteries in mouth, nose, ears. If they enter the breathing tube it can cause immediate death or bad coughing and breathlessness.
If they enter the food pipe however its a little safer. Most if the small blunt objects are harmless and come out in potty. Some sharp objects can pierce and injure the food pipe. Also small round batteries in toys are corrosive and can damage the gut. In such situations consult pediatric gastroenterologist immediately who will advise in removing these objects.
When these objects are inserted in nose or ears take to an ENT specialist immediately for removal.
Children also tend to swallow medicines, tablets, even phenyls and kerosene.
Small children are by nature curious and donot understand the consequences of their actions. Parents should always keep a close eye on them. Small objects, medicines, phenyl, kerosene should be kept locked and out of their reach.
