*HPV Vaccine*
One upbeat news that was largely missed in the beaten down Adani market (pun intended!!) ..serum institute of India has prepared our own HPV vaccine fully made in India under the brand name *Cervavac*.
It was officially announced on the girl child day 24th January at New Delhi by Adar Poonawala in presence of Home minister Amit Shaha. This brings down the cost of each unit of HPV vaccine from INR 4000 for imported type to mere 400 for desi version.
☘️: Humam Papilloma Virus is a DNA virus infecting only humans. Almost 170 types of HPV have been identified about 40 of them are sexually transmitted. HPV may also show transplacental transfer from mother to foetus.
Other notable fact is this virus is not killed by sanitizers and disinfectants.
As HPV is the most common STD, nearly every sexually active person is infected by HPV at some stage of the life.
But largely most do not show any clinical symptom.
This is mostly HPV 5 type.
HPV 1and 2 may cause common warts. Type 6 and 11 cause veneral warts and(rarely) laryngeal Papillomatosis. Warts are caused by a rapid growth of cells in outer layer of skin. Ancient Greek literature describes these warts, of course its aetiology wasn't known then.
☘️: HPV is one of the most important infectious causes of cancer. Worldwide 5%of total diagnosed cancer cases show association with sexually transmitted HPV infection. Type 16,18, 31 and 45 are labeled as high risk out of which type 16 is most notorious. It is found in nearly 47% of cervical cancers and in many cancer cases of v***a, va**na, a**s and p***s.
☘️: That ge***al HPV infection may lead to cervical cancer was first suspected and studied and proved by a German researcher Herald zur Hausen. Two French virologists too contributed to discovery of HPV. All three were awarded Nobel Prize.
☘️: In the individuals whose immune system fails to curb HPV infection can favour development of cancer. It brings about degradation of p53 protein which is identified as tumor suppressant factor in the cell. Degradation of p53 may promote unregulated cell division, cell growth and cell survival leading to cancer growth.
(Detailed theory is beyond the scope of this article. )

☘️: First HPV vaccine was made in Australia at Queensland University and approved by US FDA in 2006. Merck marketed it under trade name
Gardasil and a yr later GlaxoSmithKline marketed it as cervarix.
WHO recommends HPV vaccine as a part of routine vaccination program for all countries. It is on the list of essential medicines of WHO. Currently 125 countries are adopting it for girls aged 9 to 14( before becoming sexually active). And 47 countries are promoting its use in boys too.
☘️: Latest guidelines issued by WHO in December 2022 recommend..
1)one or two dose schedule for girls aged 9 to 14
2)similar schedule for girls between 15 to 21 yrs
3)two dose six months apart schedule for women older than 21 yrs
☘️: First generation Gardasil offered protection against HPV type 6,11,16 and 18.
In 2014 US FDA brought in Gardasil 9 which offers protection against 5 more types of HPV (total 9 types hence the name Gardasil 9)
☘️: Will the vaccine offer 100% protection against carcinoma of cervix, the answer is No. But it does offer 70% protection. Hence pap smear for cervical cancer detection is to be continued even for vaccinated women.
☘️: Public awareness about HPV vaccine in India is quite low. Lack of knowledge, not being a part of free universal immunization program, imported and hence costly are the main deterents.
The last problem is now solved. The news that was either not watched or not understood its significance.
But it surely is one more item where India has become आत्मनिर्भर!!!

If No New Variant Strikes, 3rd Covid Wave To Be Mild: Study

IIT researchers who predicted a nearly accurate peak of the Covid-19 second wave believe that the third wave if at all it comes, may not be as scary as the second one as long as no new mutant which may pass through all kinds of immunity arrives on the scene.

Their mathematical model suggests a rise in cases of infection this month in sync with the gradual opening up of various activities and that this may lead to a peak of over 1 lakh of daily cases in October in the worst-case scenario. The second wave peak had touched 4 lakh cases a day in May.

“There will be a slight increase towards the end of August and through September. It’s mainly because the lockdown is lifted. It’s not due to any new variant. There is no sign of any new mutant anywhere. So, without a new variant, the third wave will just be a ripple,” said Manindra Agrawal of IIT Kanpur.

Agrawal, involved with the national ‘super model’ initiative, known as SUTRA, on Monday said, “The third wave will certainly not be as significant as the second one. We don’t expect any scary scenario in future.”

Underlining that the peak of daily cases may be much lower (around only 50,000 cases) in absence of any new variant of Covid-19, he said, “If the third wave comes, the load on hospitals will be even lower than the first wave situation.”

The analysis and graph, released by researchers in early July, show that the overall number will eventually start tapering down after the peak in October and is most likely to reach a negligible number in December. Agrawal, however, added a caveat, saying any significant rise would happen only if a new variant comes which may pass through all kinds of immunity.

*Covid-19 Delta Variant Not Specifically Targeting Children, Says WHO*

The Delta variant of the coronavirus is not specifically targeting children, the World Health Organization said.

The WHO’s Covid-19 technical lead Maria Van Kerkhove said the evidence showed that the variant first detected in India was rather being transmitted among people who were socially mixing.

“Let me be very clear: we are not seeing the Delta variant specifically target children," the US expert told a press conference.

Delta has now been detected in 132 countries and is more transmissible than earlier versions of the virus, including other variants of concern.

The UN health agency said work was under way to gain a better understanding of the dynamics of Delta and why it is more transmissible.

“There was some suggestion that the variants were specifically targeting children, but that actually is not the case. What we are seeing is that the variants will target those who are socially mixing," Van Kerkhove said.

“What we do see is that the variants that are circulating will infect people if they are not taking the proper precautions," she said, referring to measures such as physical distancing and avoiding gathering in poorly-ventilated, crowded indoor spaces.

The WHO has published a plan detailing ways in which schools can reopen and stay open in safety.

“But we do really need to drive down transmission in the communities to make sure that they can open safely," Van Kerkhove said.

Single Shot of Covishield Offers 61% Effectiveness Against Delta Strain: Covid Panel Chief

A single shot of Serum Institute’s Covishield can provide 61 per cent effectiveness against the Delta strain as per the research, NDTV quoted Covid Working Group Chief Dr. NK Arora as saying. The Delta variant of coronavirus was first identified in India and is known to double the risk of hospitalisation. It has also been largely attributed to the massive explosion of Covid-19 cases in India during the second wave.

Meanwhile, the Union Health Ministry yesterday dismissed media reports claiming there was dissent from technical experts about increasing the gap between two Covishield doses and said the decision was based on scientific reason about the behaviour of adenovector vaccines.

The Centre on May 13 had approved extending the gap between the two doses of the Covishield vaccine from six to eight weeks to 12 to 16 weeks following a recommendation from the National Technical Advisory Group on Immunisation (NTAGI).

Based on the real-life evidence, particularly from the United Kingdom, the COVID-19 Working Group agreed to increase the interval between two doses of Covishield Vaccine to 12 to 16 weeks, the Health Ministry had said in a statement.

'Just being overweight can lead to sleep apnea', say experts

One need not be obese on the BMI scale, but just being overweight can lead to the condition of sleep apnea, say experts at AIG Hospitals on the occasion of World Sleep Day.

The condition of difficulty in breathing while asleep is increasingly becoming common due to lifestyle factors like weight gain, a study by doctors at the hospital points out. The team observed 30 patients for one month to study the phenomenon.

"A significant number of them were overweight and not necessarily obese. Yet they had obstruction in breathing. This implies that changed lifestyles are not just leading to diabetes and BP etc, but also to Obstructive Sleep Apnea," explained Dr Srinivas Kishore S, Director Department of Otorhinolaryngology.
