Yes of course! Some women unfortunately need to undergo a removal of the entire breast tissue as a safe surgical treatment for breast cancer. But, there is always an option for reconstruction.

It can either be done during the primary surgery or later after completion of entire treatment. It can be done using the patient’s own body tissues (autologous reconstruction) or using implants. Breast reconstruction is a safe procedure and does not lead to increased chances of recurrence. It in fact enhances cosmesis and significantly improves quality of life.

If not done at the time of the primary surgery, it can definitely be done once there is complete healing and adequate recovery from treatment.

Chemotherapy and hormone therapy are parts of systemic treatments of breast cancer while surgery and radiotherapy are forms of local treatments.

A lot of patients harbor doubts regarding the safety of these modalities of treatment. In spite of being slightly toxic, these treatments act only on the patient’s body. It is a myth that they impart any toxic effects on those who are near and around the patient during the course of the treatment.

In fact, being with near and dear ones during treatment not only boosts the morale of those taking treatment but also helps alleviate the side effects by positive distraction and helps in speedy recovery!

The average age at diagnosis of breast cancer in India is about 47years (as against 63yrs in the West). These days, we even get to see a lot of young women developing breast cancer. There are a separate set of issues which need to be addressed in this age group of women, one of the most important being those related to fertility.

Anticancer treatment especially chemotherapy and hormone therapy can adversely affect fertility. There are various options related to fertility preservation that need to be discussed with these young patients prior to starting treatment and appropriate steps need to be taken for the same. Also, pregnancy is contraindicated while on chemotherapy or hormone therapy.

However, after treatment, on regaining proper menstrual cycles, a normal pregnancy can be planned by the lady after a proper discussion and planning with her treating doctor.

Women are also advised to see the treating doctor once in 6 months for first 5 years after treatment and yearly thereafter, for routine physical examination.

A mammogram is recommended once in 12-18 months (of both breasts if breast conserving surgery has been done earlier or of the opposite side if a mastectomy has been done as the surgical treatment earlier). Other radiological/ blood tests are indicated only if the patient is symptomatic.

Randomized studies comparing extensive screening tests in asymptomatic women Vs routine physical examination and screening only when symptomatic, have shown no survival advantage and in fact lead to worsening of quality of life.

The treatment of breast cancer comprises mainly of surgery, radiotherapy, chemotherapy and hormone therapy (when indicated). If surgery, radiotherapy and chemotherapy are all required, the treatment roughly goes on for about 6 months. If hormone therapy is indicated, it is given for 5-10 years depending on the type of medications that can be used.

After primary treatment (surgery + radiotherapy + chemotherapy) is concluded, women are kept under surveillance. They are advised to do a breast self examination once a month and are also advised to keep a watch on certain symptoms like excessive weight loss, loss of appetite, jaundice, chronic cough not getting better with routine treatment, bony pains etc.

The most common sites of distant metastasis in case of breast cancer are liver, lungs and bones. There is no indication for doing a whole body screening in women with stage I – II breast cancer unless they show any symptoms of distant spread of disease. This is because the chances of distant spread in these stages are less than 5%.

However, all women with clinical stage III disease at presentation are screened for distant organ spread. Routine tests like chest X-Ray + Ultrasound of the abdomen and pelvis + bone scan or CT scan of the chest, abdomen, pelvis + bone scan are adequate for the same.

A PET scan is indicated very rarely in breast cancer patients. It can be used as a problem solving tool in case of ambiguous results got in the prior mentioned investigations but is not considered as an appropriate first line screening modality.

- Excessive body weight
- Lack of physical activity
- Excessive alcohol intake
- Smoking
- Prolonged excessive exposure to external hormones – oral contraceptive pills, hormone replacement therapy, treatment for infertility
- Nulliparity / delayed child bearing and lack of breast feeding
- Radiation exposure in childhood
