EUS guided gastrojejunostomy is a novel and minimally invasive daycare proedure done in patients with malignant gastric outlet obstruction. We performed it in a 45 year old male patient with inoperable tumor of left lobe of liver invading into pylorus resulting in total obstruction. Due to multiple co-morbidities, patient was a poor surgical candidate. We successfully did gastrojejunostomy by EUS and patient was discharged on next day after successful trial of solid diet.

Endoscopic Ultrasound (EUS) is an excellent modality for evaluation for Pancreatic Space Occupying malignant, premalignant and benign lesions.
Presenting a case of middle age female with incidentally detected large cystic mass in pancreas.
Cystic fluid analysis and classical morphology on EUS pointed towards Mucinous cystic neoplasm (MCN).
Patient underwent surgery and specimen histopathology confirmed the same.

ENDOSONOGRAPHY GUIDED CYSTOGASTROSTOMY.

Young male with acute on chronic pancreatitis with a pseudocyst. Initially managed conservatively, pt developed symptoms of gastric outlet obstruction with signs of infection.
We drained the psedocyst endoscopically with drainage of 1 litre of pus like fluid on table. Pt was discharged on post endoscopy day 3. Complete pseudocyst resolution achieved on day 10 in follow up CT scan.
Endoscopic drainage is totally painless and scarless technique for draining pancreatic pseudocysts.

Chronic pancreatitis is a painful debilitating condition, which leads to very poor quality of life, requiring daily analgesics, oral as well as IV.
Presenting a case of idiopathic chronic pancreatitis with dilated pancreatic duct with stricture in head in PD. We did stricture dilatation, calcific flecks removal followed by PD stenting. Patient is pain free and off analgesics for 1 month so far.
Identifying ideal candidates for pancreatic endotherapy is most important.
