મેટાસ એડવેન્ટિસ્ટ હોસ્પિટલ( મિશન હોસ્પિટલ) સુરત માં ફરી એક વાર કેેેડેેવર ટ્રાન્સપ્લાન્ટ થયું ...

54 વર્ષીય કૈલાસબેન મહેન્દ્રભાઈ પટેલ સુરત ના રહેવાસી છે. તેમનું બીજીવાર કીડની ટ્રાન્સપ્લાન્ટ 2018 પછી 5 વર્ષ બાદ પાછું સફળ અને સુરક્ષિત કેડેવર કિડની ટ્રાન્સપ્લાન્ટ તારીખ 15/5/2023 ના રોજ થયું છે. બીજીવાર કેડેવર કિડની ટ્રાન્સપ્લાન્ટ કરી કૈલાસબેન ને બીજીવાર નવજીવન મળ્યું છે.

કૈલાસબેન મહેન્દ્રભાઈ પટેલ ને 2010 માં કિડની ની તકલીફ થયેલી ત્યારબાદ કૈલાસબેન ને તેમના સગા ભાઈ એ 2011 માં કિડની આપી હતી તે કિડની 2018 માં કામ કરતી બંધ થઇ ગઇ અને પાછા ડાયાલિસિસ પર આવી ગયા ત્યાર બાદ અમદાવાદ IKDRC માં નામ નોંધાવ્યું. તેમણે 3 વર્ષ સુધી કિડની મળી નોહતી.દરમિયાન મા કૈલાસબેન નો સંપર્ક ડૉ વત્સા પટેલ સાથે થયો અને કૈલાશબેને એમનું નામ અમદાવાદ થી સુરત માં મેટાસ એડવેન્ટિસ્ટ હોસ્પિટલ( મિશન હોસ્પિટલ) સુરતમાં ટ્રાન્સફર કરાવ્યું. કૈલાસબેન ને જ્યારે પણ કીડની માટે નો કોલ આવતો ત્યારે એમણે બધી તૈયારીઓ સાથે અમદાવાદ જવું પડતું હતું. કૈલાસબેન ની તબિયત નાદુરસ્ત રહેતી હોવાથી કૈલાસબેને તેમનુ નામ સુરત માં મેટાસ એડવેન્ટિસ્ટ હોસ્પિટલ( મિશન હોસ્પિટલ) સુરત માં કરાવ્યું હતું. કૈલાસબેન ને 11 વખત કિડની માટે કોલ મળ્યા પણ કૈલાસબેને હાર ન માની અને 12 મી વખત તારીખ 14/5/2023 રવિવાર ના રોજ પાછો કૈલાસબેન ને કિડની માટે નો કોલ મળ્યો. અને આ વખતે ડૉ વત્સા પટેલ દ્વારા પ્લાઝમાફેરેસીસ, ઈમ્યુનોગ્લોબ્યુલીન, ડબલ ડ્રગ ઇમ્યુનોસપ્રેસન વગેેરે આપીને એન્ટી એચએલએ એન્ટિબોડીઝને દૂર કરવાના બહુવિધ પ્રયાસો થકી કૈલાસબેન કિડની મેચ કરવા માટે નસીબદાર બન્યા. આ કીડની કૈલાસબેનને સોટો (SOTTO), ગુજરાત મારફતે રાજકોટમાંથી મળી.

યુરોલોજિસ્ટ ડૉ. કપિલ ઠક્કર, ડૉ. નરેન્દ્ર પારેખ અને ડૉ. આશિષ ગોટી દ્વારા અને એનેસ્થેટિક ડૉ. ધવલ વરિયા દ્વારા ઈમરજન્સીમાં 16 કલાકના લાંબા સમય કિડની બરફમાં રહી હોવા છતાં સફળતાપૂર્વક ટ્રાન્સપ્લાન્ટ કરવામાં આવ્યું હતું.
આ ઓપરેશન દરમ્યાન ઘણી બધી મુશ્કેલીઓ બહુવિધ ધમનીઓ અને ઝરતા ઘાને કારણે આવી.છતાં પણ ખુબજ સારી રીતે સફળતા પૂર્વક ઓપરેશન પાર પાડવા માં આવ્યું. કૈલાસબેનને 1.4 ક્રિએટીનાઇન સાથે ખુશી ની લહેર સાથે 10 દિવસ પછી રજા મળેલ છે.

Till now 10 successful transplants has been done including this complicated cadaver transplant!!

A 54 year female with anti GBM disease, had chronic allograft nephropathy ( kidney failure after transplant) after 11 years of LRKT( live related kidney transplant) (brother was donor) presented to us with 98%PRA ( meaning she will get 2% chance of getting match amongst 100 kidney donors) with high MFI( higher level of antibodies) against multiple HLA antigen while awaiting second kidney transplant for 5 years.
Multiple attempts done to remove anti HLA antibodies by giving IVIG, plasmapheresis , double drug immunosuppression while awaiting transplant by
*Dr Vatsa Patel ,* Nephrologist and transplant physician. After 11 times found positive crossmatch, ultimately recieved successful cadaver donor transplant on 12th attempt,she got lucky to get kidney matched after plasmapheresis. Successfully transplanted in emergency by urologist Dr Kapil Thakkar, Dr Narendra Parekh and Dr Ashish Goti with anesthesia given in emergency by Dr Dhaval Varia. She also got surgically difficult anastomosis because of multiple arteries and oozing wound ,very well managed and finally she discharged home happily with creatinine of 1.4 after 9 days inspite of long cold ischemia time ( kidney remained in ice) of 16 hours.Kidney recieved from Rajkot via SOTTO, Gujarat.

Kudos to Metas Adventist Hospital team for doing 10th Successful transplant headed by transplant physician Dr Vatsa Patel, nephrologist Dr Anil Patel, urologist team Dr Kapil Thakkar, Dr Chirag Patel, Dr Narendra Parekh, Dr Ram Patel.
Anesthesia by Dr Rajiv Pradhan, Dr Yuvraj Chauhan, Dr Dhaval Varia. Without help of Dr Himanshu Mandirwala, Dr Bhavi Shah as radiologist; Dr Harnish Badami and Dr Frenil munim as pathologist and microbiologist, it was not possible.
