Not all foot ulcers in diabetics are diabetic ulcers

This patient had come to us 4 years back with a diabetic heel ulcer with osteomyelitis which we were able to heal.He presented to the OPD with a dorsal foot ulcer of 3 weeks duration in the region of seemingly eczematous skin that was being treated by dermatologists. We excised the ulcer and out of suspicion sent it for biopsy .The report came as well differentiated squamous cell carcinoma. The ulcer was reexcised with frozen section to obtain negative margins followed by an anterolateral thigh free flap reconstruction by our oncoplastic microvascular surgeon.

Animal bites and Genomic sequencing/screening Assay

This patient presented to us with severe pain and swelling of the right foot with discharging sinus at the site of cat bite. Was being treated at home with oral antibiotic. We took her up for exploration of the foot and found some Slough and purulent discharge which was tracking along the tibialis anterior.Cultures from the wound were sterile.Post operatively the wound showed features of delayed healing / infection with recurrent edema ,wound dehiscence and f/s/o cellulitis .Infectious disease specialist was consulted and was advised genomic screening of tissue to identify organisms that are found in animals but cannot be detected on tissue cultures like Vitek.Corynebacterium were detected and treated with culture specific antibiotics that accelerated the healing process and resulted in wound healing .
