FINGER INJURIES /HAND INJURIES

The fingers are the most common site of injury, both in work related and in domestic accidents. The injuries can range from minor cuts and bruises to deep cuts injuring the deeper structures. The major cuts may have a loss of tissues and involve bony fractures.

One of the most common causes of finger injuries (seen across all age groups) is when fingers get jammed between doors. As a reflex action we tend to pull back our fingers and this causes more damage and can even lead to loss of the jammed part.

Another common cause is cut injuries, while handling sharp objects like knives, chisels etc or even cuts caused by the blade of a kitchen mixie. Animal bites can also cause finger injuries.

Finger injuries also commonly occur during accidents caused while doing home repairs, especially while using a hammer. Hitting the hans or the finger instead of the nail, will cause a crush injury.

The common presentation of a finger injury includes cuts with bleeding, bruising, bluish discoluration of nails, pain, swelling, deformity of fingers and difficulty in moving the finger.

FIRST AID

* Wash the injured part in clean running
water

* If there is bleeding , elevate the finger
ABOVE THE LEVEL OF THE HEART and
apply pressure with a clean cloth for 10
minutes

* If there is no active bleeding apply an ice
wrapped in a thin cloth to the injured
part

* Take a painkiller medicine and if required
antibiotic ointment can be applied to the
cuts.

* If the finger is jammed between doors try
to open the door rather than pulling your
finger away as it causes less damage

DO NOT

* DO NOT forcibly remove any material that
is embedded in the wound

*DO NOT try to straighten any deformity

ALWAYS CONSULT THE DOCTOR IMMEDIATELY

* In all cases of animal bites

* When the bleeding cannot be controlled

* If the cut is deep and underlying strucures
are seen

* If there is numbness or deformities

* If after few days after the injury you
develop pain redness or fever always
consult the doctor as it could be a sign of
infection

BURNS AWARENESS - PART 1

I remember a patient who had sustained burns on her hand and had come to my OP in a panic, a few years ago. The reason this particular patient stays in my memory is because ... as she walked in, the whole OP was engulfed in what can only be described as the smell of mint ! The burnt area was covered in white paste . When I tried checking the burn, she said she had applied toothpaste as first aid for the burn.
Later on patients have come to me with their burn wounds covered with common things like toothpaste (Yes there were more) tea powder, honey, turmeric and even the uncommon dosa batter!
I started asking these patients who came after applying toothpaste to the burnt area as to why they did it. Some of them said that it was the first cream-like substance they got. Some thought it would give a cooling effect. Few others had heard from others that it was the best first aid for burns !

What then is the right thing to do in burns?

Do

* Wash the burnt area in running tap water at room temperature for 20 minutes

* After washing, cover the area with a clean cloth and get medical attention.

* In very small areas of burn after washing ,you can apply antibiotic ointment

Don't

*Do not apply ice,ice cold water, honey, toothpaste etc. to the burnt area

* Do not break any blisters that appear

*Do not forcibly remove any clothing that is stuck to the burnt area

* Do not apply any creams or ointments when shifting to the hospital. It will be easier for the doctor to assess the burn depth if it is not covered with creams.

Burns can be of many kinds... some more common than others. I will try and explain the various kinds of burns and their treatment in my next post.

FOOT CARE IN DIABETICS

In diabetics ( as part of the diabetes ) patients develop neuropathy or altered sensation of the feet. Initially it presents as burning or tingling of the feet and slowly progresses to loss of sensation. This combined with reduced blood flow can lead to infections, ulcers and even gangrene if left untreated or ignored. Therefore care of the feet is very important for diabetics.

Do's and don'ts

*Inspect your feet at the end of the day. Look for any sores, corns or bruises. You can use a mirror to examine the sole. Always examine the space between your toes

* Wear correct fitting footwear. Ill fitting footwear can cause sores

* Keep the space between your toes dry to prevent infections

* Regular walking is good for your feet

* Apply moistoriser to prevent dryness

* Never cut corns or calluses by yourself. Always consult a doctor

* Never use hot water bags or boiling hot water to soak your feet as it may get burnt. Use your elbow to check the temperature before soaking your feet, sunce your feet may not be able to sense the correct temperature.

* Never walk barefoot. Get your doctor's opinion regarding any special footwear

* Take care when cutting nails. Never cut too close to the skin. It is always easier to cut after soaking in water.

* If there's an ulcer ,follow your doctor's advice regarding management of the ulcer.

Regular inspection of the feet , following the dos and avoiding the don'ts can help diabetics in keeping their feet healthy in the long run.

Please feel free to clarify any queries you might have.

Carpal Tunnel Syndrome

If you have carpal tunnel syndrome you could possibly have one of the following symptoms :

* Tingling sensation in your fingers when
you work
* Difficulty to pinch or hold keys
* Dropping objects from your hand
frequently

The median nerve as it passes from the wrist to the palm, passes through a narrow tunnel called the carpal tunnel. Due to repetitive work like using the computer mouse the nerve gets compressed within the canal.

Initially, it presents as tingling sensation in the thumb, index and middle fingers. If left untreated, it can lead to more numbness and weakness of the muscles, making it difficult to hold objects.

The doctor can diagnose this condition by clinical examination and running a few tests.

Treatment can be non surgical by wearing splints, analgesics or even steroid injections.

Surgical treament is done by, dividing the band compressing the nerve under local anesthesia. This surgery is done as a day case procedure, and you can return back to work in about 2 to 3 days.

If you have any doubts about this procedure please message me.

WHAT IS PRP ?

PRP or platelet rich plasma treatment is an effective method to reduce hair loss and hair thinning.

It is done as an outpatient procedure and the procedure takes less than an hour only.
For preparing the PRP, blood is collected in special tubes and centrifuged.

The PRP is then injected into the areas of hair loss and thinning. This is rich in growth factors which promote healthy growth of hair follicles.
