Nasal polyps

Nasal polyps are common, affecting up to 4 % of population. Their etiology remains unclear, but they are known to have associations with allergy, asthma, infection, cystic fibrosis and aspirin sensitivity.
These are benign lesion arising from the mucosa of the nasal sinuses or from the mucosa of the nasal cavity. They are smooth, round, semi-translucent masses that are most commonly found in the middle meatus and ethmoid sinuses.

Elevated level of histamine and IgE found around polyps, mast cell and esinophilia found within the polyps provide evidence suggesting the inflammation is a major factor in polyp formation.

Causes

The mucosa is very wet layer that helps protects the inside of your and sinuses and humidify the air you breathe. During an infection or allergy induced irritation, the nasal mucosa becomes swollen and red and it may produce fluid that drips out with prolonged irritation the mucosa may form a polyp.

Triggers :

• Chronic or recurrent infection
• Cystic fibrosis
• Allergy
• Hay fever
• Asthma

Symptoms

Common sign and symptoms of chronic sinusitis with nasal polyp include:
• Loss of sense of taste
• Snoring
• Frequent node bleeds
• Post nasal drip
• Running nose
• Headache
• Obstructive sleep apnea
• Persistent stuffy or blocked nose

Treatment

In ayurveda, nasal polyps are correlated with nasarasha. Once the sinuses are infected, improper lifestyle can leads to the disease in chronic phase. The nasal polyp is correlated with kapha dosa which accumulated in the wall of blood vessels and surrounding muscle tissue and lead to swelling.

Medical management of polyps is the same as that for allergic rhinitis and aimed at relieving symptoms until surgery can be arranged, but it usually remains incomplete. If the surgery is performed under local anesthesia the polyp themselves often shrink, which makes removal difficult.
1. Nasya :
Herbal powder /oils or herbs are administered to nostril to stimulate secretion of mucosa and thereby dissolve and loosen mucus lodged in nasal passage.
2. Steam inhalation :
One drop of camphor or turmeric .steam should be inhaled twice
3. Dhoomapanam:
Nasal passage can be cleared and reduce kapha by inhaling the fumes of turmeric.
4. Ksharakrma
Ksharam of medicinal plants applied over the medial surface of exposed turbinate, by using the cotton rolled over jobson probe. Applied kshara was kept for 2 min. After some time the colour changed to reddish brown. The kshara was then removed with cotton rolled with probe. Lemon juice was applied over the turbinate to neutralize the remaining kshara. Grhita applied over the burnt wound to overcome daha.

RHUEMATOID ARTHRITIS

Rheumatoid arthritis is a chronic disease of the joints that can cause a disabling degree of pain and significantly interfere with an individual’s day to day activities.

RA is a worldwide health issues and its chronic inflammatory disease it causes progressive joint damage and even leads to disability .its onset is closely related to environmental and genetic factors.
Over the past 2 decades the treatment of RA has been revolutionized by advances in its pathological mechanism and development of drug which target them.
RA mainly attack the joints usually, many joints at once. RA commonly affects in the hands,wrist and knee .In a joint with RA,the lining of joint become inflamed,causing damage to joint tissue.This tissue damage can cause long lastin or chronic pain unsteadiness and deformity.

Causes

RA is an autoimmune disease .your immune system is supposed to attack foreigners in your body like bacteria,viruses by creating inflammation. In an autoimmune disease ,the immune system mistakenly send inflammation to your own healthy tissue. The immune system creates a lot of inflammation that is sent to your joints causing joint pain and swelling. If the inflammation remains present for a long period of time ,it can cause damage to joint. This damage typically cannot be reversed once it occur.The cause of RA is unknown.

Signs and symtoms

When the disease is active ,RA symptoms and sign includes.
• Fatigue
• Low grade fever
• Loss of energy
• Joint redness
• Joint pain and warmth
• Deformity
• Loss of range of motion
Complications
• Anorexia
• Bad taste in mouth
• Polyurea and burning micturition
• Colicky pain
• Thirst ,fainting,giddiness and pericardial discomfort

Deformities:

Early in the disease, RA tends to attack the small joints of your hands and feet. This can lead to several types of deformities, including:
Ulnar drift. Your fingers lean away from your thumb and toward your pinky.
Boutonnière deformity. Also called a buttonhole deformity. Your middle finger joint bends downward (toward your palm), while your top finger joint bends away from the palm.
Swan-neck deformity. The base and top joint of your finger bend downward, but the middle joint is straight.
Hitchhiker’s thumb: Also called a z-shaped deformity. Your thumb flexes at the joint where it meets your palm, then bends backward at the joint below your thumbnail.
Bunion. The base of your big toe (where it attaches to the foot) gets larger and sticks out. This may force your big toe to press against the second toe, forcing it to overlap the third.

TREATMENT

Rheumatoid arthritis has been described as Amavata in ayurveda,in which the vitiated vata and ama afflicts the lining f joints ,causing shula and s***a
The treatment module for amavata includes shamana and shodana chikitsa .Depending upon the stages and doshas involved ,many measures are advised such as langana(a method of depletion of body tissues),deepana(increase digestive fire),pachana(digestion of food) for accumulated ama snehapana(oleation),swedana (fomentation),virechana(purgation) and basti (e***a) for elimination of doshas from the body.
Basic principles of treatment
1. Abyanagam :
Is a form of ayurvedic medicines that involves massage of body with dosha specific warm herb infused oil.
2.Patra pinda swedanam :
Swedanam done with the fresh leaves of vata pacifying herbs .This bundle of vata pacifying herbs and choornas is rubbed on the affected part gently. Circular motion with gentle pressure is given over the joints and rubbed vertically and give fomentation over muscles on back, shoulder and hand.
3.upanaham: Ekanga sweda done with preapared vatahara leaves and mix with medicated choornas and decoction of herbs. The affected part cleaned and mild abyanga should be done locally and part covered with this medicated paste and tie with bandage cloth and usually done in the night.
4.Dhara :Stream pouring of medicinal liquids mainly dhanyamla dhara.

CERVICAL SPONDYLOSIS

Cervical spondylosis is an age related wear and tear affecting the spinal disk in the neck.it also called as arthritis of neck or degenerative or cervical osteoarthritis of neck.
The outcome of neck pain depends on the underlying cause, but acute neck pain usually resolves within days or weeks ,although it can reoccur or become chronic. In the cervical spine this chronic degenerative process affects the intervertebral disks and facet joints. Normally soft disks between vertebrae, these disk become compressed.
Causes
Spinal disc may slowly wear down with time and disc become thinner and soft tissue has less elasticity. When cartilage in joint of vertebrae start to degenerate and the bone tissue rub directly against other bone tissues and abnormal bone growth develop along with the edges of vertebrae.
As a result of these abnormalities ,cause narrowing of spinal column which called as cervical spinal stenosis.
Symptoms
Most of the people don’t have any significant symptom, if do occur ,they can vary from mild to severe and may develop gradually or suddenly.
• Pain over shoulder and arms
• Neck stiffness and pain
• Grinding sound or sensation when the neck is turned or rotated
• Headache
• Numbness in shoulder and hands
• Feel loss of balance and loss of bladder or bowel control

TREATMENT
In Ayurveda greevasthi can be correlated as cervical spondylosis which is a disorder of vayu. Vyana vayu is responsible for the movement of body.
Greevavasthi is one among 80type of vatavyadhi which is characterized by stamba (stiffness) in neck region which is commonly seen in cervical spondylosis and also considered as grivagata sandivata especially in degenerative condition.in this Ayurveda treatment is mainly focused to pacify the vitiated vata dosha.

COVID AND AYURVEDA

Pandemic is not now new to india.In all those pandemic situations ,ayurveda our ancient treatment procedures had shown its reliability upto the mark rather than any other medications.Towards the end of 2019,a novel coronavirus was spreaded all over the world wide. Ayurveda being one of the oldest surviving medical tradition ,has a key role to play in this crucial situation.
From ayurvedic point of view COVID 19 is a epidemic disease .Being the science of life,propogates the gift of nature in maintaining health and happy life
The national protocols says that the detailedrecommendations are based on empirical evidences,biological plausibility and knowledge from ayurveda classics and experience from clinical practices.
We all know that prevention is better than cure.While there is no medicines for covid 19 as of now,it will be good to take preventive measures which boost our immunity in these pandemic situation. Ayurveda focused on the concepts of dinacharya –daily regimen and ritucharya –seasonal regimen to maintain health.

HEEL PAIN

Heel pain can be a symptom of many medical conditions. It can affect our normal day to day activities. Heel pain is a general term used to describe pain and discomfort experienced in or around the rear of the foot. There are many reasons for heel pain that can induce mild to moderate and severe heel pain.

Most common causes:
• Plantar fasciatis –inflammation of plantar fascia .it’s a ligament from calcaneum (heel bone) to the tip of the foot. When it get stretched too much its soft tissues fiber become inflamed. Pain usually starts when you take the first step in the morning, gradually depending on the movement pain normally decreases at the same time prolonged standing or walking results the pain in return. People who are overweight and those who wear improper shoes have increased risk of this condition.
• Achilles tendinitis: Due to the overuse of band tissue that connect the calf muscle to the back wide of the heel bone. It can be healed with simple medications and self care
• Heel spur: Due to the stress when put over the plantar fascia ligament .It does not caused only by this, but cause a spur over heel where the plantar fascia attaches to heel bone. Heel spur may not be the cause of heel pain even when it shows on x ray.
• Bursitis: It affect the heel, causes pain in the middle of the undersurface of the heel that often worsens with prolonged standing and pain of heel worsen if the patient bend his foot up /down.
• Fractures: Due to repetitive stress, strenuous exercise,sports or heavy works lead to fracture in the metatarsal bone of the foot.

Treatment

In ayurveda the treatment of heel pain depends on the severity of the pain which patient is suffering. Moreover treatment loosing weight,warming up, stretching ,wearing fit shoes ,self care measures will reduces heel pain.
The pain in mainly manifested due to the aggrevated vata,these cause blockages in srotas (channels) and if excess ama is accumlated in the body cause heel pain .
1.lepanam : Lepanam is the application of medicated paste over the heel to reduce the vitiated vata doshas.
2.upanaham: Ekanga sweda done with preapared vatahara leaves and mix with medicated choornas and decoction of herbs. The affected part cleaned and mild abyanga should be done locally and part covered with this medicated paste and tie with bandage cloth and usually done in the night.
3.Snehana:oil massage
4.swedana-rooksha –especially ishtika sweda which is the easiest form of treatment.
4.Agnikarma: Best treatment modality, which help to pacify s***a and shoola by the opposite quality such as ushna ,tikshna,ashukari(quick acting).In this ,therapeutic heat make effects to twak and gradually to deeper structure which done with the help of redhot panchaloha shalaka which pacify ama dosha.

Tennis elbow is a classic repetitive strain injury, also a painful condition of the elbow caused by overuse. It’s also known as lateral epicondylitis. Tennis elbow is a common yet sometimes complex musculoskeletal condition affecting many patient treated by physical therapists. Recent studies on injuries of tennis players has helped to expound the facts about the type of injuries that athletes sustain. This term widely used, but the condition should be understood as not limited to tennis players.
Patients complain of an area of pain and tenderness over the bony prominence of lateral epicondyle of humerus.This structure is also the common origin of the long extensor tendons for the forearm ,hand and the underlying pathology appears to be an area of degenerative changes within these tendons due to repeated strain injuries.

Symptoms
• pain slowly aggravates around the lateral portion of elbow, less often may develop suddenly
• pain from gripping and movements of wrist especially extension & lifting movements
• pain and weakness –shake hands, grip an object and daily activities

causes
This condition implies the presence of an inflammatory condition, which present only in the earliest stages of the disease process. This cause is repeated contraction of forearm muscle that to straighten and raise your hand and wrist. The repeated motion and stress to tissue results in two series of tears in tendon that attach the forearm muscle

Common:

Treatment:
According ayurveda snayugata vata can be correlated with this condition.Snayugata vata is developed when vata dosha aggrevates due to ativyayama and atichesta which localized in snayu of koorpara sandhi . Hypothyroidism is the most common thyroid disorder where more frequently women are affected than men. This disorder can be either primary or secondary where primary hypothyroidism men’s that thyroid gland itself is diseased ,while secondary hypothyroidism caused by problems in pituitary gland.

Unfortunately hypothyroidism is often remains undiagnosed or misdiagnosed because they are non specific in nature in which they mimic symptom of other disease & common symptoms includes:

Cause

Treatment

In ayurveda hypothyroidism can correlate with kapha dosa,rasa and medovaha strotas as well as manovaha strotas.
The objective of treatment should be to address the problem at its root by regulating the immune system and decreasing inflammation. Srotosodhana (cleansing of macro and micro channels),agnideepanam(stimulation of the digestion and metabolism),pachana(digestion)and vatanulomanam(proper regulation of excretory system) are the main principles of treatment.
1. Nasya : Hypothyroidism is an urdhwajatrughata vyadhi so nasya plays an important role in here so as it is indicated. Nasya with herbs which are thyroid stimulating are very useful, and can achieve strotoshodana.
2. Vamana: vamana is to eliminate kapha dosa associated with pitta. Hypothyroidism is kapha vridhasamana and pitta vridhi so it helps to eliminate obstructions.
3. Virecanam :It is the cleansing method performed by giving herbal purgatives which acts on dhatwagni level and corrects agni for pachana.
4. Vasti: Is an effective ayurvedic treatment bringing cleansing of subtle channels or srotsas spread all over the body.
5. Takradhara: Hypothyroidism is an psychosomatic disorder so takradhara with aragwadadi ,varunadi,eladigana is beneficial.

SINUSITIS
Sinuses are a connected system of hollow cavities in the skull. They reduces the weight of the skull and they humidify the inspired air.
Sinusitis is the inflammation of lining inside these sinuses, which can be acute or chronic. These infections occur when the mucous membrane, It swell and become inflamed.
on the basis of location -4 pair called paranasal sinus.
• Frontal – above eyes in forehead
• Ethmoid - cheekbones, under eyes
• Maxillary – between eyes ,nose
• Sphenoid – centre of skull, behind nose and eyes.

TYPES

• Acute : short duration
: Symptoms usually last less than 4 weeks.
• Sub acute : Symptoms last between 4 – 12 weeks.
• Chronic : More than 12 weeks.
• Recurrent :Four or more episode each lasting for 10 days
with absence of symptoms between episodes.
CAUSES

Sinusitis can stem from various factors :
• Nasal polyp –small growth in the lining of nose. These tissue growth can block the nasal passage.
• Deviated nasal septum -restrict or block the nasal passage.
• Allergies such as pollutants, dusts
• Respiratory tract infections-RTI inflame and thickens the sinus and block mucus drainage. These infections can be viral, bacterial or fungal.

SYMPTOMS

Common signs and symptoms:
• Facial pain and pressure
• Nasal congestion and cloudy discharge
• Anosmia
• Halitosis (bad breath)
• Sore throat by nasal drip
• Cough –body’s attempt to clear out throat irritant

MANAGEMENT

According ayurveda sinusitis can be considered as kaphaja siroroga and can deal with various type of external and internal therapeutics. Ayurveda suggests following a healthy regimen that does not lead to the aggravation of doshas and mainly kapha dosa is responsible for dosa of phlegm.

1. Nasya : one among the five ayurvedic procedure,mainly recommended for shiroroga which elimante dosas by administered herbal juice,tailam and powderinto the nostrils.
2. Thalam: paste with medicated oil applied over head.
3. Dhoomapanam: Apply the paste of haridra choorna over the cloth and make dried & inhale the fumes. it clear the pathway and reduces kapha dosas.
4.Kavala gandusha : Gargling with medicated water.
5.jalaneti and kapalabheti
6.pranayams and yogasanas

OBESITY
From the perspective of Ayurveda, obesity is the accumulation of excessive fat and flesh(muscle tissues).The key foundation for the cause of obesity is incorrect lifestyle ,diet choice or habit.
Absence of physical activities, high intake of sugar, junk foods, daytime sleep, salty or oily food etc.. are also the root cause of obesity. Increasing the amount of bio toxins will affect the metabolic process to form more fatty tissues and block the formation of tissues with bones. The unhealthy formed tissue layer increase unwanted fats and cause disturbance in the functioning of kapha.

• How is obesity measured ?
Body Index Mass (BMI) is a value derived from the mass and height of the person. The BMI is divided the square of body height, and is universally expressed in units of kg/metre square

• BMI ABOVE 30 Obese

Symptoms
