Dr Siva Kumar Darimireddy sir successfully performed a Transcatheter Aortic Valve Replacement (TAVR).

Patient Profile

A 60-year-old male with multiple comorbidities including Diabetes Mellitus, Hypertension, Chronic Kidney Disease, and COPD.

Procedural Challenge

The left coronary artery had a very low origin, with a coronary height of only 5 mm from the aortic annulus, which posed a significant procedural challenge.

However, the presence of a wide Sinus of Valsalva measuring 38 mm and a Valve-to-Coronary (VTC) distance of 12 mm allowed safe deployment of a 24.5 mm Myval Transcatheter Heart Valve, resulting in a successful outcome.

Intravascular Lithotripsy (IVL) uses controlled sonic pressure waves to fracture calcium deposits inside coronary arteries, allowing the vessel to expand properly before stent placement.

Inside this artery was something harder than plaque ..calcium.
Calcium makes arteries rigid, resistant to balloons, and difficult to expand during angioplasty.

Rather than forcing the artery open with very high pressure, IVL modifies the calcium first, improving vessel compliance and enabling optimal stent expansion.

🔬 Clinical insight:
IVL is particularly effective for deep and concentric coronary calcification, where conventional balloon angioplasty may fail.

Answer for the recently posted Question

In post-CABG CASES, is it appropriate to administer Inj, Tirofiban ?

In SVG (saphenous vein graft) lesions, the plaque inside the graft is usually soft and fragile (friable). When we manipulate it during PCI, small pieces can break off and travel downstream, causing blockages in multiple small vessels.

Tirofiban (a GP IIb/IIIa inhibitor) mainly works by preventing platelet aggregation at one main site of injury. This works better in native coronary arteries, where the problem is usually a rupture of the vessel lining at one fixed spot.

In SVG cases, since the issue is debris and embolization happening in multiple areas, Tirofiban may not fully address the problem.

Historically, Interventionists are cautious with GP IIb/IIIa inhibitors in elderly patients and especially in those who recently had surgery because of the increased bleeding risk.
