𝗕𝗲𝘆𝗼𝗻𝗱 𝗔𝘂𝗱 con Lukas Rizzardi e Sara Fioretti è stato ospite di Starkey Italy S.r.l. in una giornata dedicata all’innovazione per gli audioprotesisti, dal tema: 𝗟’𝗼𝘁𝘁𝗶𝗺𝗶𝘇𝘇𝗮𝘇𝗶𝗼𝗻𝗲 𝗱𝗲𝗹 𝗳𝗶𝘁𝘁𝗶𝗻𝗴 𝗱𝗶 𝗢𝗺𝗲𝗴𝗮 𝗔𝗜 𝗮𝘁𝘁𝗿𝗮𝘃𝗲𝗿𝘀𝗼 𝗹’𝘂𝘁𝗶𝗹𝗶𝘇𝘇𝗼 𝗱𝗶 𝗿𝗲𝘃𝗼𝗹𝗼𝘂𝗱.

Abbiamo approfondito il software 𝗿𝗲𝘃𝗼𝗹𝗼𝘂𝗱 di Hörzentrum Oldenburg gGmbH, partendo dai concetti cardine di loudness, parlando dei metodi di loudnessscaling, dello studio di validazione di revoloud e di concetti come la BBLS.

Oggettivare la soggettività, dando il giusto peso alle 𝗽𝗲𝗿𝗰𝗲𝘇𝗶𝗼𝗻𝗶 𝗱𝗲𝗹 𝗽𝗮𝘇𝗶𝗲𝗻𝘁𝗲 per garantire un comfort uditivo senza precedenti.

È stato stimolante riscontrare un clima di grande apertura e confronto. Vedere così tanto interesse verso tematiche audiologiche innovative ci conferma che la strada è quella giusta.

Heute geht es um die zweite Phase im Versorgungsmodell:

2️⃣ 𝗗𝗶𝗲 𝗪𝗮𝗵𝗹 𝗱𝗲𝘀 𝗛ö𝗿𝘀𝘆𝘀𝘁𝗲𝗺𝘀: 𝘸𝘪𝘦 𝘰𝘳𝘪𝘦𝘯𝘵𝘪𝘦𝘳𝘵 𝘮𝘢𝘯 𝘴𝘪𝘤𝘩 𝘣𝘦𝘪 𝘏𝘶𝘯𝘥𝘦𝘳𝘵𝘦𝘯 𝘷𝘰𝘯 𝘔𝘰𝘥𝘦𝘭𝘭𝘦𝘯?

Wir fokussieren uns auf vier grundlegende Punkte:

𝟭. 𝗔𝘂𝗱𝗶𝗼𝗹𝗼𝗴𝗶𝘀𝗰𝗵𝗲𝗿 𝗕𝗲𝗱𝗮𝗿𝗳
Die Auswahl beginnt bei den technischen Merkmalen, die erforderlich sind, um das spezifische Hördefizit bestmöglich auszugleichen.

𝟮. 𝗔𝗻𝗮𝘁𝗼𝗺𝗶𝗲
Die Form des Gehörgangs und die Morphologie der Ohrmuschel sind entscheidend. Ein System muss langfristigen Tragekomfort, Sitzstabilität und eine korrekte Interaktion mit den natürlichen Resonanzen des Gehörgangs gewährleisten.

𝟯. 𝗙𝗲𝗮𝘁𝘂𝗿𝗲𝘀
Wir betrachten dieaudiologische Gesamtsituation. Wenn akustisch komplexe Situationen eine stärkere Unterstützung erfordern, kommen folgende Aspekte ins Spiel:
• Störschallunterdrückung, Richtmikrofone und Deep Neural Networks (DNN) für ein intelligentes Management komplexer Klangumgebungen
• Wireless-Systeme und Konnektivität zur Unterstützung bei akustische hearusfordernden Situationen

𝟰. 𝗧ä𝗴𝗹𝗶𝗰𝗵𝗲 𝗡𝘂𝘁𝘇𝘂𝗻𝗴 𝘂𝗻𝗱 𝗛𝗮𝗻𝗱𝗵𝗮𝗯𝘂𝗻𝗴
Die Hörsysteme sollen eine Hilfe sein, kein (technisches) Hindernis. Die Gerätewah muss die muss den Anforderungen des Patienten entgegenkommen:
• Energieversorgung: Abwägung, ob ein Einwegbatterie-System oder eine wiederaufladbare Lösung funktionaler für den Nutzer sind
• Design: Berücksichtigung spezifischer Lösungen für Kinder, sehbehinderte Nutzer oder Personen, die eine vereinfachte Handhabung benötigen

Today we move on to the second pillar:

The shape of the ear canal and the morphology of the pinna are decisive. A device must ensure long-term comfort, positioning stability, and correct interaction with the ear’s natural resonances.

We look at the complexity of the auditory system. When signal audiological situation requires greater support, several factors come into play:
• adaptive noise reduction, directional microphones and Deep Neural Networks (DNN), for intelligent management of challenging acoustic environments
• wireless systems and connectivity, to support those facing more pronounced audiological challenges.

𝟰. 𝗗𝗮𝗶𝗹𝘆 𝗨𝘀𝗲 𝗮𝗻𝗱 𝗠𝗮𝗻𝗮𝗴𝗲𝗺𝗲𝗻𝘁
Technology must be an ally, not an obstacle. The choice must promote patient autonomy:
• evaluating whether a disposable battery or a rechargeable system is more functional for the user’s habits.
• considering specific solutions for pediatric patients, visually impaired users, or those requiring simplified management, turning maneuverability into a strength of the entire prosthetic journey.

Guardiamo alla complessità del sistema uditivo centrale. Quando l’analisi del segnale richiede un supporto maggiore, entrano in gioco:
• Automatismi e Deep Neural Networks (DNN), per una gestione intelligente degli ambienti sonori complessi.
• Sistemi Wireless e Connettività, per supportare chi presenta sfide audiologiche più marcate.

𝟰. 𝗨𝘀𝗼 𝗤𝘂𝗼𝘁𝗶𝗱𝗶𝗮𝗻𝗼 𝗲 𝗚𝗲𝘀𝘁𝗶𝗼𝗻𝗲
La tecnologia deve essere un alleato, non un ostacolo. La scelta deve favorire l'autonomia del paziente:
• Alimentazione, valutare se il sistema a batteria usa e getta o quello ricaricabile sia più funzionale alle abitudini dell'utente.
• Design inclusivo, considerare soluzioni specifiche per l'età pediatrica, per utenti con deficit visivi o per chi necessita di una gestione semplificata, trasformando la manovrabilità in un punto di forza dell'intero percorso protesico.

Wie bereits beim Start der Serie angekündigt, kann der Weg zu einer erfolgreichen Anpassung nicht auf ein solides Fundament verzichten.
Heute beginnen wir mit der ersten, grundlegenden Säule:

Oft als lästige Routineuntersuchung betrachtet, ist die tonaudiometrische Schwellenmessung der erste Schritt zu einer erfolgreichen Hörgeräteversorgung.
Es geht nicht nur darum, die minimale wahrgenommene Intensität zu bestimmen, sondern eine fundierte Grundlage für jeden weiteren Anpassungsschritt zu messen.

Die Tonaudiometrie ist der Grundpfeiler unserer klinischen und diagnostischen Tätigkeit und ermöglicht die Bestimmung der Hörschwelle.

Warum wir sie nicht als selbstverständlich ansehen sollten:

• Diagnostik: Sie ermöglicht es uns zwischen Schallleitungs-, sensorineuralen oder kombinierten Hörverlusten zu unterscheiden.

•Maskierung: Eine korrekte Anwendung der Maskierung ist essentiell für eine korrekte Diagnostik und verhindert grobe (Mess)Fehler.

• Grundlage für die Anpassung: eine ungenaue Audiometrie ist die Vorstufe einer erfolglosen Hörgeräteanpassung. Die vorberechneten Verstärkungswerte (ob NAL-NL2 oder DSL v5) sind nur “gut” wenn die Audiometriedaten korrekt sind.

Often considered a routine test, pure-tone threshold audiometry is the first predictive step toward hearing aid benefit. We are not just looking for the minimum perceived intensity; we are defining the scope of our intervention.

Pure-tone audiometric testing remains the cornerstone of our clinical and diagnostic activity, allowing us to define sensitivity thresholds.

Why we should never take it for granted:

• Diagnostic accuracy: it is the first dividing line between conductive, sensorineural, or mixed hearing loss.
• The value of masking: managing it correctly ensures clinical precision and prevents major errors.
• Foundation for fitting: inaccurate audiometry is the perfect recipe for a failed hearing aid fitting. Prescriptive gain (whether NAL-NL2 or DSL v5) is only as effective as the input data.

L’esame audiometrico tonale rimane la pietra miliare della nostra attività clinica e diagnostica, che permette di definire la soglia di sensibilità.

Perché non dobbiamo darlo per scontato:

• Precisione diagnostica: è il primo spartiacque tra ipoacusie trasmissive, neurosensoriali o miste.
• Il valore del mascheramento: gestirlo correttamente è precisione clinica che evita errori grossolani.
• Fondamento per il fitting: un’audiometria imprecisa è la ricetta perfetta per un adattamento protesico fallimentare. Il guadagno prescrittivo (che sia NAL-NL2 o DSL v5) è efficace solo quanto lo sono i dati di input.

𝗢𝗹𝘁𝗿𝗲 𝗶𝗹 𝗴𝗿𝗮𝗳𝗶𝗰𝗼: 𝗶𝗹 𝗳𝗮𝘁𝘁𝗼𝗿𝗲 𝘂𝗺𝗮𝗻𝗼

2️⃣ 𝗚𝗲𝗿ä𝘁𝗲𝗮𝘂𝘀𝘄𝗮𝗵𝗹: Welche technischen Merkmale wirklich wichtig sind und welche Schritte zur "richtigen" Wahl führen.
3️⃣ 𝗔𝗻𝗽𝗮𝘀𝘀𝘂𝗻𝗴 & 𝗙𝗶𝘁𝘁𝗶𝗻𝗴: Die Anwendung von Anpassformeln und welche Daten einfließen
4️⃣ 𝗩𝗲𝗿𝗶𝗳𝗶𝘇𝗶𝗲𝗿𝘂𝗻𝗴: Instrumente und Methoden zur Überprüfung der Funktionalität der Hörsysteme

2️⃣ 𝗗𝗲𝘃𝗶𝗰𝗲 𝗦𝗲𝗹𝗲𝗰𝘁𝗶𝗼𝗻: how to navigate product features and how to select the "correct" device
3️⃣ 𝗗𝗲𝘃𝗶𝗰𝗲 𝗔𝗽𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 & 𝗙𝗶𝘁𝘁𝗶𝗻𝗴: the application of prescriptive formulas and their specific characteristics.
4️⃣ 𝗩𝗲𝗿𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻: tools that allow you to verify the functionality and tecnical effectiveness of the fitting.
5️⃣ 𝗩𝗮𝗹𝗶𝗱𝗮𝘁𝗶𝗼𝗻: examinate the outcome of the fitting by testing speechunderstanding, loudness and listening effort, questionairs and patient feedback.

3️⃣ 𝗔𝗽𝗽𝗹𝗶𝗰𝗮𝘇𝗶𝗼𝗻𝗲 𝗱𝗶𝘀𝗽𝗼𝘀𝗶𝘁𝗶𝘃𝗼 𝗲 𝗳𝗶𝘁𝘁𝗶𝗻𝗴 𝗶𝗻𝗶𝘇𝗶𝗮𝗹𝗲: l'applicazione delle formule prescrittive e le loro caratteristiche.
